摘要
目的观察慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭患者温阳法治疗前后T淋巴细胞亚群的动态变化及其临床疗效。方法将112例COPD合并Ⅱ型呼吸衰竭患者随机分为两组,均采用有创机械通气与西医常规治疗,治疗组加用三脏温阳饮。两组分别于治疗前、治疗后2周、4周测定外周血CD3+、CD4+、CD8+含量和CD4+/CD8+的变化,并观察其机械通气时间和呼吸机相关肺炎(VAP)发生率。结果治疗组治疗2、4周后CD3+和CD4+T淋巴细胞绝对计数和CD4+/CD8+明显高于对照组;CD8+T淋巴细胞绝对计数明显低于对照组。治疗组VAP发生率明显低于对照组。结论三脏温阳饮能改善COPD合并Ⅱ型呼吸衰竭患者的细胞免疫功能,其临床疗效确切,并能减少VAP发生率。
Objective： To observe the clinical effect and the dynamic change of T Lymphocyte Subsets for pa- tients suffering from chronic obstructive pulmonary disease （COPD） with type-Ⅱ respiratory failure pre and post treatment. Methods： 112 COPD patients with type-Ⅱ respiratory failure were randomly divided into two groups, and both groups were given invasive mechanical ventilation together with regular therapy,treatment group was added Sanzangwenyang Decoction. The level of CD3＋,CD4＋,CD8＋ in the Peripheral Blood were determined and the change of CD4＋/CD8＋ before and after 2 weeks and 4 weeks treatment were observed. And observe mechanical ven- tilation time and change in incidence of VAP also were observed. Results： The absolute counts of CD3＋ and CD4＋ T lymphocyte and CD4＋/CD8＋ in treatment group which was treated 2 or 4 weeks were significantly higher than those of control group.The absolute counts of CD8＋ T lymphocyte in treatment group were significantly lower than those of control group.The incidence of VAP in treatment group was obviously lower than that of control group. Conclusion： Sanzangwenyang Decoction can improve cellular immune function of COPD patients with type- Ⅱ respiratory failure, reduce the incidence of VAP, and the clinical effect is definite.
