定制钢板治疗胫骨平台骨折的初期疗效探讨 被引量:3

Initial efficacy of custom plates in the treatment of platform fracture

摘要 目的探讨定制钢板治疗胫骨平台骨折的方法和初期疗效。方法 2011年3月~2011年12月,7例胫骨平台骨折患者采用定制钢板治疗。根据骨折的AO分型,B3型2例,C1型2例,C2型2例,C3型1例。入院后行骨折部位三维CT扫描(层厚0.7 mm),数据通过Mimics 10.01软件分析,再经过计算机辅助设计(CAD)制作个性化钢板,局部消肿后行有限切开定制钢板内固定术。结果随访时间8~14个月,平均12.3个月。骨折愈合时间12~35周,平均12.8周。无骨不愈合病例。膝关节应用Neer关节功能评分标准评分,优5例,良1例,中1例,差0例。优良率达85.7%。结论定制钢板结合微创技术治疗胫骨平台骨折具有良好的疗效。 Objective To explore the making and initial effects of custom plates in the treatment of platform fractures. Methods From March to December 2011, 7 patients with platform fractures were treated with custom plates. According to AO fracture classification, 2 cases were of type B3, 2 cases were of type C1 and C2, and 1 case was of type C3. After CT scanning (0.7mm per slice), three - dimensional reconstruction and operation mimesis were conducted with Mimics 10.01. The plates and screws were designed and made by the means of computer - assisted design (CAD). The plates contoured perfectly to the reduced fracture area and mimicked for virtual inter- nal fixation. Techniques of minimally invasive plate osteosynthesis (MIPO) were used for the treat- ment of 7 cases during operation. Results The average follow-up duration was 12.3 months, rang- ing from 8 to 14 months. The average time of fracture healing was 12.8 weeks, ranging from 12 to 35 weeks. No ununion was found during the folio-up. According to scoring system based on the oc- currence of fracture site, the results were evaluated as excellent in 5 cases, good in 1 case, fair in 1 case and poor in 0 case. The rate of excellent and good was 85.7 %. Conclusion Virtual intro- op- eration internal fixation could be mimicked vividly. The satisfied curative effects could be achieved with custom plates and MIPO techniques for platform fractures.
