达芬奇机器人系统在直肠癌根治术中的应用(附70例报告) 被引量:16

摘要 目的:探讨达芬奇机器人系统治疗直肠癌的安全性及有效性。方法:回顾分析2010年6月至2011年10月为70例直肠癌患者行达芬奇机器人直肠癌根治术的临床资料。结果:70例均顺利完成手术,无一例中转开腹,无吻合口漏、吻合口狭窄发生,手术时间平均(220.2±50.3)min,系统装配时间平均(10.1±5.4)min,术中出血量平均(55.8±20.2)ml,术后胃肠功能恢复时间平均(2.6±1.6)d;标本残端均无肿瘤细胞残留,平均清扫淋巴结(17.1±4.2)枚;术后平均住院(4.5±2.1)d。术后1例发生骶尾部切口脂肪液化,2例骶尾部切口感染,1例尿潴留,2例肠梗阻,均经保守治疗后好转。随访至2011年10月,患者生活质量良好。结论:达芬奇机器人技术应用于直肠癌根治术安全可行,疗效显著。 Objective:The purpose of this study was to discuss the safety and feasibility of robot-assisted resection of rectal cancer.Methods:The clinical data of 70 patients who underwent robot-assisted resection of rectal cancer from Jun.2010 to Oct.2011 was retrospectively analyzed.Results:All of the patients were operated successfully with no conversion,anastomotic leakage,anastomotic stenosis.The average operative time was(220.2±50.3) min.The mean time of system assembly was(10.1±5.4) min.The mean intraoperative blood loss was(55.8±20.2) ml.The mean postoperative gastro intestinal function recovery time was(2.6±1.6) d.All specimens stump were found with non-cancer cell infiltration,number of harvested lymph node was(17.1±4.2).The average hospital stay after surgery was(4.5±2.1) d.Postoperative complications occurred in 6 patients,including 1 case of fat liquefaction and 2 cases of infection of sacrococcygeal region incision,1 case of urinary retention and 2 cases of intestinal obstruction.All complications took a favorable turn after conservative treatment.Until Oct.2011 the life quality of all patients were good.Conclusions:Robot-assisted resection of rectal cancer is safe,feasible and effective.
