Harmonic focus ultrasonically activated scalpel in open thyroid surgery

摘要 目的探讨harmonic focus超声刀在甲状腺癌外科治疗中的临床应用价值,分析超声刀在甲状腺癌手术中的临床优越性。方法回顾性分析2009年10月~2011年4月在我科接受手术的甲状腺癌177例的临床资料,90例采用超声刀治疗(超声刀组),87例采用电刀治疗(传统手术组)。采用统计学检验比较两组的癌灶大小、总手术时间、腺叶切除时间、术后引流量、引流管放置时间、术后并发症发生率、术后甲状旁腺激素及血清钙磷水平变化。结果与传统手术组相比,超声刀组明显缩短了总手术时间和腺叶切除时间(P<0.01),但二者在术后引流量、引流管放置时间、术后并发症发生率、术后甲状旁腺激素及血清钙磷水平变化方面无明显差异(P>0.05)。结论甲状腺癌外科治疗采用超声刀技术,不仅节约了手术时间、简化手术操作,而且对周围组织创伤小,不增加手术并发症、术后引流量及引流时间,值得临床推广使用。 Objective To discuss the clinical value and superiority of harmonic focus scalpel in thyroid cancer surgery. Methods A retrospective analysis was performed on 177 patients who bad been given total thyroidectomy plus functional neck dissection from September 2009 to April 2011. The patients were divided into two groups: 90 were operated by use of the harmonic focus scalpel( the harmonic focus group ) and 87 by use of the mono-polar elec- tric scalpel(the traditional group). Statistical test was used to compare , lump diameter,total operation time, time of thyroid lobectomy, post-operative drainage volume, indwelling time of drainage tube and post-operative complications, pre-operative and post-operative parathyroid hormone (PTH) and serum Ca and P between the two groups. Results Compared with the traditional group, total operation time and time of thyroid lobectomy were significantly decreased in the harmonic focus group ( P 〈 0.01 ), while the post-operative drainage volume, indwelling time of drainage tube, post-operative complications, pre-operative and post-operative parathyroid hormone (PTH) and serum Ca and P( P 〉 0.05 ) were not. Conclusion Harmonic focus scalpel in thyroid cancer surgery can not only decrease the operation time,simplify the surgical procedure,but also reduce the trauma to the surrounding tissues, at the same time does not increase post-operative drainage volume, indwelling time of drainage tube and post-operative complications. Harmonic focus scapel as a new surgical instrument deserves to be widely used.
