摘要
目的探讨乳腺浸润性微乳头状癌(Invasive micropapillary carcinoma,IMPC)的临床病理学特征。方法应用光镜和免疫组化染色法对3例IMPC进行分析,并复习文献。结果 3例IMPC光镜下特征性表现为肿瘤细胞呈桑椹状、微乳头状或小腺管样,巢与周围间质形成明显的空隙;免疫组化染色EMA在瘤细胞中有独特的阳性表达,即在癌细胞巢团外表面EMA呈膜阳性表达;E-Cadherin表达于肿瘤细胞间连接面的细胞膜,而朝向间质面的细胞膜则表达消失。结论 IMPC是一种少见类型的浸润性乳腺癌,淋巴结转移率高,预后差。
Objective To investigate the clinicopathologic characteristics of invasive micropapillary carcinoma （IMPC） of the breast. Methods 3 cases of IMPC of the breast were studied by histopathology and immunohistochemical techniques. The clinical data and the related literatures were reviewed. Results No special clinical situations were found in 3 cases. Histologically, the characteristic performance of the tumor cells usually show morule-like cluster and central lumens patten ,the micropapillary pattens were also can be seen. There was obvious interspace between cancer nests and peritumoral stroma. The tumor nest was detected by an outer membranous immunostaining pattern of EMA in tumor tufts. E-cadherin was expressed in the membrane of contiguous surfaces among carcinoma cells but not in the membrane toward the stromal surfaces. Conclusion IMPC is a rare type of invasive breast cancer,with a high incidence of lymph node metastases and poor prognosis.
