尿白蛋白肌酐比值和胱抑素C预测冠脉狭窄程度价值研究 被引量:2

The forecast value of urine albumin-to-creatinine ratio and cystain C on coronary artery stenosis severity

摘要 目的探讨尿白蛋白肌酐比值(UACR)和胱抑素C(Cys C)对冠脉狭窄程度的预测价值。方法选择2009年9月至2011年10月在右江民族医学院附属医院经冠脉造影(CAG)确诊为冠心病的患者159例,分为稳定型心绞痛(SAP)组46例、不稳定型心绞痛(UAP)组75例、急性心肌梗死(AMI)组38例,同时选取健康成人40名为对照组,检测入选者清晨空腹血糖、血脂、Cys C及UACR,明确患者冠脉病变支数和冠脉Gensini积分情况,比较各组UACR和Cys C及其与冠脉病变的相关性。结果冠心病3个亚组中UACR明显高于对照组,SAP组和UAP组中Cys C也明显高于对照组(P均<0.01),而AMI组中Cys C反而低于对照组(P<0.05),在冠心病组中双支病变组患者UACR和Cys C明显高于单支病变组(P<0.01),而多支病变组患者UACR和Cys C也显著高于双支病变组(P<0.01),UACR和Cys C与冠脉Gensini积分呈明显的正相关(r分别为0.647、0.652,P均<0.01)。结论 UACR和Cys C与冠状动脉病变程度密切相关,联合检测UACR和Cys C可初步预测冠状动脉狭窄程度。 Objective To explore the forecast value of Urine Albumin-to-Creatinine Ratio and Cystain C on coronary artery stenosis severity. Methods 159 patients with coronory heart disease diagnosed by Coronarography were chose as CHD group, and divided into three Subgroups,in which there are 46 patients of stable angina pectoris (SAP) ,75 of unstable angi- na pectoris(SAP) and 38 of acute myocardial infarction(AMI) ,and 40 healthy people were chose as control group. To de- termine the fasting blood-glucose, blood fat, Cys C and UACR for all the subjects. All the CHD patients were definite known diseased coronary vessels by CAG and coronory Gensini integral. To compare UACR and Cys C among every group and its relationship with diseased coronary vessel. Results The UACR of three subgroups of CHD are significant higher than that in eontorl group (P 〈0. 01 ) ,so does Cys C in SAP group and UAP group than control group(P 〈0. 01 ) ,but Cys C is lower in AMI group(P 〈 0. 05 ). The UACR and Cys C of double diseased coronory vessels are all higher than single diseased ( P 〈 0. 01 ) , and which in many diseased coronory vessels group is higher than the former( P 〈 0. 01 ). There is obviously positive correlation between UACR and Cys C level and coronary Gensini integral( ris 0. 647,0. 652 ,P 〈 0. 01 ). Conclusion UACR and Cys C are close relative to severity of coronary artery, combinated determination of UACR and Cys C can preliminary forecast of coronary artery stenosis degree.
