磁共振成像在子宫内膜癌术前分期及肌层浸润深度判定中的作用(依照新修订的FIGO分期系统) 被引量:17

The value of preoperative MRI in staging and assessing the depth of myometrial invasion of endometrial carcinoma(according to new revised FIGO staging system)

摘要 目的:评价在新修订的FIGO分期系统下,磁共振成像(magnetic resonance imaging,MRI)在子宫内膜癌术前分期及肌层浸润深度判定中的作用。方法:对36例子宫内膜癌进行术前MRI分期和肌层浸润深度判定,并与手术病理分期对照。结果:MRI术前分期诊断准确率为91.7%(33/36)。MRI诊断无肌层侵犯、浅肌层侵犯和深肌层侵犯的敏感性、特异性、准确率分别为50%、85.7%、77.8%;84.4%、76.5%、80.6%;80.0%、100%、94.4%。MRI区分Ⅰa期(无肌层侵犯和浅肌层侵犯)和Ⅰb期(深肌层浸润)的诊断准确率为94.4%(34/36)。结论:MRI对子宫内膜癌术前分期及肌层浸润深度的判断准确率较高,具有很高的应用价值。 Objective:To evaluate the role of magnetic resonance imageing(MRI) in preoperative assessment of the staging and the depth of myometrial invasion of endometrial carcinoma.Methods: Thirty six cases of endometrial carcinoma were examined by MRI before operation,and the pathological findings were compared with MRI findings.Results:The accuracy of MRI in staging endometrial carcinoma was 91.7%(33/36).The sensitivity,specificity and accuracy of the MRI imaging in assessment of non-myometrial invasion,superficial myometrial invasion,deep myometrial invasion of endometrial carcinoma were 50%,85.7%,77.8%;84.4%,76.5%,80.6%;80.0%,100%,94.4%,respectively.The accuracy for distinguishing stagesⅠa andⅠb was 94.4%(34/36).Conclusion:MRI is of high value in assessing the depth of myometrial invasion and staging in patients with endometrial carcinoma.
