摘要
目的探讨3.0磁共振(MRI)在进展期子宫内膜癌术前诊断中的临床应用价值。方法对2018年1月-2019年1月期间齐齐哈尔市中医医院收治的经手术病理证实为子宫内膜癌患者62例的临床资料进行回顾性分析,所有患者均接受了MRI平扫加强+弥散加权成像(DWI)检查,并以手术病理诊断结果作为“金标准”,比较MRI对子宫内膜癌诊断的准确率,并计算其特异性、敏感性、阳性预测值以及阴性预测值。结果经手术病理证实本次研究所纳入的62例患者中子宫内膜癌ⅠA期的有39例,ⅠB期的有9例,Ⅱ期的有5例,ⅢA期的有1例,ⅢB期的有4例,ⅢC1期、ⅢC2期、ⅣA期以及ⅣB期各1例,以此作为诊断金标准,MRI+DWI诊断的整体准确率达到了96.77%(60/62),P>0.05,特异性为100%(1/1),敏感性为96.72%(59/61),阳性预测值为100%(59/59),阴性预测值为33.33%(1/3)。结论3.0磁共振对于进展期子宫内膜癌诊断具有较高的应用价值,可作为临床可靠的影像学参考依据。
Objective To investigate the clinical value of 3.0MRI in preoperative diagnosis of advanced endometrial carcinoma.Methods The clinical data of 62 patients with endometrial carcinoma confirmed by operation and pathology in our hospital from January 2018 to January 2019 were analyzed retrospectively.All patients underwent enhanced MRI and diffusion weighted imaging(DWI),and the results of operation and pathology were used as the“gold standard”to compare the accuracy of MRI in the diagnosis of endometrial carcinoma.The specificity,sensitivity,positive predictive value and negative predictive value were calculated.Results Among the 62 patients included in this study,there were 39 cases of stageⅠA,9 cases of stageⅠB,5 cases of stageⅡ,1 case of stageⅢA,4 cases of stageⅢB,1 case of stageⅢC1,1 case of stageⅢC2,1 case of stageⅣAn and 1 case of stageⅣB.As the diagnostic gold standard,the overall accuracy of MRI+DWI diagnosis reached 96.77%(60/62),P>0.05,the specificity was 100%(1/1),the sensitivity was 96.72%(59/61),the positive predictive value was 100%(59/59),and the negative predictive value was 33.33%(1/3).Conclusion 3.0 MRI has high application value in the diagnosis of advanced endometrial carcinoma and can be used as a reliable imaging reference in clinic.
