摘要
目的探讨Snail mRNA与VEGF mRNA在乳腺浸润性导管癌中的表达及其临床意义。方法采用原位杂交技术检测30例乳腺单纯性增生、30例乳腺导管内癌和70例乳腺浸润性导管癌中Snail mRNA及VEGF mRNA的表达水平,比较两种基因在乳腺不同病变组织中和在乳腺浸润性导管癌不同病理特征组间表达的差异,以及两种基因表达的相关性。结果 乳腺单纯性增生、乳腺导管内癌和乳腺浸润性导管癌组织中SnailmRNA表达阳性率分别为23.3%(7/30)、46.6%(14/30)和81.4%(57/70),其差异有统计学意义(χ2=32.4,P<0.05);VEGF mRNA表达阳性率分别为33.3%(10/30)、50.0%(15/30)和71.4%(50/70),其差异也有统计学意义(χ2=13.4,P<0.05)。在乳腺浸润性导管癌组织中,Snail mRNA和VEGF mRNA表达阳性率在有淋巴结转移者中明显高于无淋巴结转移者〔92.7%(38/41)比65.5%(19/29);85.4%(35/41)比51.7%(15/29)〕,差异均有统计学意义(χ2=8.29,P<0.05;χ2=9.42,P<0.05);Snail mRNA和VEGF mRNA表达阳性率在TNMⅢ~Ⅳ期中高于Ⅰ~Ⅱ期〔93.9%(46/49)比52.4%(11/21);81.6%(40/49)比47.6%(10/21)〕,差异均有统计学意义(χ2=14.14,P<0.05;χ2=8.32,P<0.05);Snail mRNA和VEGF mRNA的表达还与ER、PR及HER-2的表达和血管癌栓有关(P<0.01),而二者的表达与患者的年龄、肿瘤大小和组织学分级无关(P>0.05);Snail mRNA表达与VEGF mRNA表达呈正相关(r=0.67,P<0.05)。结论 Snail和VEGF在乳腺浸润性导管癌组织中过度表达对其的发生、发展可能起协同作用,联合检测Snail和VEGF mRNA表达对预测乳腺浸润性导管癌浸润、转移有一定意义。
Objective To investigate the expressions of Snail and VEGF gen e in invasion ductal carcinoma tissues and analyze their clinicopathologic relat ionship.Methods The expressions of Snail and VEGF gene were detected on mamma ry gland hyperplasia(30 cases),intraductal breast cancer(30 cases),and invasion ductal carcinoma(70 cases) by in situ hybridization,to compare with th e expression difference of the two genes in the different pathological changed t issues of mammary gland and among the clinicopathological facters of invasion du ctal carcinoma as well as the relationship.Results The expre ssion rate of Sn ai mRNA in mammary gland hyperplasia,intraductal breast cancer,and invasion ductal carcinoma was 23.3%(7/30),46.7% (14/30),and 81.4%(57/70),respectively,there was statistical di fference among them(χ2=32.4,P0.05);The expre ssion rate of VEGF mRNA in mammary gland hyperplasia,intraductal breast cance r,and invasion ductal carcinoma was 33.3%(10/3 0),50.0%(15/30),and 71.4%(50/70),respectively,there was statistical difference among th em(χ2=13.4,P0.05).The expre ssion rates of Snail mRNA and VEGF mRNA in lymphatic metata sis group were significantly higher than those in no lymphatic metatasis group (92.7%(38/41) vs.65.5%(19/29),χ 2=8.29,P0.05;85.4%(35/41) vs.51.7%(15/29),χ2=9.42,P0.05,respectively).The expressi on rates of Snail mRNA and VEGF mRNA in Ⅲ-Ⅳ stage of TNM clinical stage were significant ly higher than those in Ⅰ-Ⅱ stage(93.9%(46/49) vs.52. 4%(11/21),χ2=14.14,P0.05;81.6%(40/49) vs.47.6%(10/21),χ2=8.32,P0.05).The expressions of Snail mRNA and VEGF mRNA were related to the expressions of ER,PR,HER-2,and vessel cancer embolus(P0.01).The expressions o f Snail mRNA and VEGF mRNA were not related to age,tumor size,and histologic al grade(P0.05).There was a positiv e correlation between the expressions of Snail mRNA and VEGF mRNA(r=0.67,P0.05).Conclusions The over-expressi ons of Snail mRNA and VEGF mRNA in invasion ductal carcinoma has a synergetic effect o n occurrence and development,therefore,combined detecting the expressions of Sn ail mRNA and VEGF mRNA are some significance to predict infiltration and metastasis o f the invasion ductal carcinoma.
