摘要
目的:观察320例广西地区慢性肾小球肾炎肾脏病理与中医证型间关系。方法:对320例长期居住广西并经肾活检的慢性肾小球肾炎患者进行肾脏病理及中医辨证分型的回顾性分析。结果:320例慢性肾小球肾炎患者中,以系膜增生性肾小球肾炎最多,其次是IgA肾病;主证以脾肾气虚证型最多见,其中有240例有兼证,从多到少依次为血瘀证、水湿浸渍、湿浊中阻、外感、湿热壅盛;中医主证与病理分型间具有关联性(P<0.05),辨证为肺肾气虚、脾肾气虚的患者多见于系膜增生性肾小球肾炎;中医兼证与病理类型间存在关联性(P<0.05)。血瘀证在IgA肾病中最多见;中医辨证各组尿素氮、肌酐相比无明显差异(P>0.05);各组在总胆固醇、甘油三酯、血浆白蛋白、24 h尿蛋白定量方面比较有差异。结论:慢性肾小球肾炎各病理类型与中医证型具有相关性,临床上进行中医辨证分型对推测慢性肾小球肾炎的病理类型具有一定的意义。
Objective： To observe the correlation between renal pathology typing and TCM syndrome of 320 cases of chronic glomerulonephritis in Guangxi province.Methods： 320 long-term residents with chronic glomerulonephritis in Guangxi province were given a retrospective analysis of renal pathology and syndrome differentiation type.Results： Among 320 patients with chronic glomerulonephritis,patients with mesangial proliferative glomerulonephritis occupied the first place,followed by IgA nephropathy.The most common type of main syndromes was the syndrome of qi deficiency of the spleen and kidney,of which 240 cases had additional syndromes,the decreasing order was blood stasis syndrome,soaking syndrome,syndrome of dampness and turbid stagnated in the middle energizers,syndrome of obstructed exopathogens,dampness and heat;there was a correlation between TCM main syndrome and pathology typing（P0.05）,patients differentiated with syndrome of qi deficiency of the spleen and kidney are more likely to have mesangial proliferative glomerulonephritis;there was a correlation between TCM additional syndrome and pathology typing（P0.05）,blood stasis syndrome was the most common in IgA nephropathy;there was no significant difference（P0.05） in urea nitrogen and creatinine in TCM groups;there were great differences in total cholesterol,triglycerides,serum albumin,24 h urinary protein of all groups.Conclusion： There is a correlation between various pathological types of chronic glomerulonephritis and TCM syndromes,and the clinical syndrome differentiation has certain significance on the speculation of pathological type of chronic glomerulonephritis.
