摘要
目的探讨2型糖尿病湿热困脾证患者在人群分布及空腹C肽分泌表现方面的特征。方法对151例首发2型糖尿病病例按照《中药新药临床研究指导原则(试行)》之"中药新药治疗糖尿病的临床研究指导原则"进行中医辨证分型,对比不同证型间的男女构成比例、年龄、体质量指数(BMI),同时测定空腹血浆C肽含量,对比不同证型间血浆C肽水平。结果湿热困脾证男性比例及BMI显著高于其他证型(P<0.05),女性比例及发病年龄显著低于其他证型(P<0.05或P<0.01);湿热困脾证空腹血浆C肽水平显著高于其他证型(P<0.01),且空腹C肽分泌分型以高分泌型为主。结论 2型糖尿病湿热困脾证具有一定的人群特征,以年轻肥胖男性为主;高分泌型特点显示湿热困脾证患者普遍存在胰岛素抵抗或胰岛素利用效率低下的病理特点。
Objective To investigate the characteristic of crowd and secretion characteristics of fasting C-peptide of TCM syndrome of damp-heat affecting the spleen of type 2 diabetes mellitus patients. Methods151 patients with type 2 diabetes mellitus were differentiated into 5 TCM syndromes and fasting C-peptide in blood plasma was determined.Compared the sexual percentage,age,body mass index（BMI） and the level of C-peptide of 5 TCM syndromes. ResultsPercentage of male and BMI of TCM syndrome of damp-heat affecting the spleen were obviously higher than others syndromes（P0.05）,percentage of female and age of onset were obviously lower than others syndromes（P0.05 or P0.01）,the level of fasting C-peptide in blood plasma was obviously higher than others syndromes（P0.01）,and the secretion type of fasting C-peptide was given priority to hypersecretion type.ConclusionTCM syndrome of damp-heat affecting the spleen of type 2 diabetes mellitus has certain characteristics of crowd,and gives priority to young obese males.The characteristic of hypersecretion type shows that the patients with TCM syndrome of damp-heat affecting the spleen have the pathological characteristics of insulin resistance or low utilization rate of insulin.
