摘要
目的:观察静脉滴注抗菌药物对幽门螺杆菌(H pylori)的影响,探讨静脉滴注抗菌药物对上胃肠道疾病影响的临床意义。方法:观察对象需同时具备以下三个条件,①有需要静脉滴注抗菌药物的疾病;②有上胃肠道疾病;③H pylori阳性。共选择46例,分为A组和B组,各23例,两组静脉滴注的抗菌药物相同,A组加服多潘立酮,B组加服奥美拉唑,疗程7 d,停药4周后复查胃镜。结果:有38例患者完成了诊疗计划,每组19例。H pylori根除率,A组为52.6%,B组为89.5%,两组对比差异具有统计学意义(P<0.05);胃镜下病变好转率,A组为68.4%,B组为94.7%,两组对比差异具有统计学意义(P<0.05)。结论:静脉滴注抗菌药物对H pylori具有一定的根除作用,也有引起耐药菌株产生的可能,只有患者患有其他疾病需要静脉滴注抗菌药物时,此时可以和口服奥美拉唑等药物联合根除H pylori。
Objective： To observe the impact of intravenous drip of antibacterials on helicobacter pylori （H pylori）, to discuss the clinical significance of the impact of intravenous drip of antibacterials on upper gastrointestinal diseases. Methods： The objects chose had three conditions simuhaneously, （1）had the disease which need intravenous drip of antibacterials; （2）had upper gastrointestinal disease; （3）postive for helicobacter pylori. A total of 46 patients, divided into two groups, group A and group B, each group had 23 patients, intravenous drip of the same antibacterials in two groups, group A added domperidone to be taken orally, group B added omeprazole to be taken orally, treatment 7 d, and four weeks later take stomachoscopy a- gain. Results： 38 patients completed the treatment program, 19 cases in each group. The eradication rate of H pylori was 52.6% in group A and 89.5% in group B, there was a significant difference between two groups （P〈0.05）; the improvement rate of the disease through stomachoscopy was 68.4% in group A and 94.7% in group B, there was a significant difference between two groups（P〈0.05）. Conclusion： Intravenous drip of antibacterial is effect on eradicating H pylori in a certain ex- tent, also have caused drug-resistant strains of may, only patients suffer from some diseases needed intravenous drip of antibacterials,we can combine intravenous drip of antibacterials and omeprazole to be taken orally to eradicate H pylori.
