摘要
目的观察皮质类固醇对儿童反复发作过敏性紫癜(Henoch-Schonlein purpura,HSP)的治疗作用。方法 2005年8月～2008年10月我科收治反复发作HSP患儿74例,患儿随机分成甲基强的松龙治疗组(42例)和常规治疗对照组(32例)。两组患儿在病程中均采用常规治疗,包括:合并感染加抗感染治疗、潘生丁、维生素C、钙剂、西替利嗪糖浆。在此基础上治疗组选用甲基强的松龙(Methy-prednisolone,MP)1～2mg/(kg.d)静脉用药,4～6d后改为泼尼松1mg/(kg.d),分3次服用,每隔5d～7d减2.5mg～5mg,至逐减完为止,疗程3周～4周。结果治疗组皮疹消退、腹痛缓解、关节症状消退及肾损害恢复时间明显短于对照组(P<0.01),紫癜复发率(7.14%)明显低于对照组43.75%(P<0.01)。结论皮质类固醇可有效治疗儿童HSP,预防儿童HSP复发。且有一定减轻肾损害作用。
OBJECTIVE To study of therapeutic effects of the corticosteroids on repeated attack of Henoch-Schonlein purpura in pediatric.METHODS 74 cases were divided into two groups：the 32 cases of controlled group of routine treatment and the 42 cases of treated group of the Methy-prednisolone.RESULTS The treated group was significantil improved in symptom include tetter,abdominal pain,symptom of alimentary tract and recovery of kidney damage（P〈0.01）.The recurrence rate of puroura in the treated group（7.14%）was significantiy less than that the controlled group（43.75%）.CONCLUSIONS The therpy has good effect concerning repeated attack of Henoch-Schonlein purpura treated with corticosteroids in pediatric,it has the better treatment in preventing recurrence,and has some reducing kidney damage.
