软、硬通道多靶点微创术并颅压监测救治大量脑出血的研究 被引量:2

摘要 目的探讨救治大量脑出血安全、有效、理想的治疗方法。方法将符合入选条件的146例大量脑出血患者,按治疗方法分为软、硬质通道联合多靶点微创术并颅压监测(ICP)治疗组(A组)84例,软通道微创术对照组(B组)62例,两组术后对比研究。结果 A组术后ICP较术前明显改善(P<0.01);A组再出血率明显低于B组(P<0.01);A组血肿清除率明显高于B组(P<0.001);意识程度评分(GCS)A组明显高于B组(P<0.01),A组神经功能缺损降低百分数(%)较B组明显(P<0.01);A组有效率明显高于对照(P<0.05);A组病死率明显低于B组(P<0.05)。结论应用软、硬质通道联合多靶点微创术并术中持续监测颅内压治疗大量多发脑出血,可减少再出血等并发症,提高血肿清除率,降低病死率,改善患者预后,提高患者的生存质量。 Objective To explore one safe, effective, ideal method for curing massive cerebral hemorrhage method of treatment. Methods Conform to the selected condition 146 patients who have massive cerebral hemorrhage, according to the method of treatment divides into two group, treatment group ( A group) of 84 patients with soft and flinty channel to unite multi- target micro to create in the technique and the technique monitors the ccrebrospinal fluid pressure (ICP), Control group (B group) of 62 patients with the soft channel micro creates the technique , the contrasts research of using two different techniques. Results Through therapy, the ICP of A group was distinct improvement comparing before the surgery ( P 〈 0. 01 ) ;the more hemorrhage rate of A group is lower than B group obviously ( P 〈0. 01 ) ; the haematoma percentage clearance of A group is higher than B group obviously (P 〈 0. 01 ) ;the consciousness degree grading (GCS) of A group is higher than B group obviously (P 〈 0. 01 ) ; the reduction of nerve function damage percentage of A group is more obvious than B group ( P 〈 0. 001 ) ; the effectiveness of A group is higher than the control group obviously ( P 〈 0. 05 ) ; the mortality rate of A group is lower than B group obviously ( P 〈 0.05 ). Conclusion Applies soft and flinty chanael to unite multi - target micro to create in the technique and the technique monitors the cerebrospinal fluid pressure treatment massively to send cerebral hemorrhage continually, may reduce bleeds and so on complications again, raises the haematoma percentage clearance, cuts the case fatality rate, improves the patient prognosis, improves patient's survival quality.
