摘要
目的:将高强度聚焦超声(HIFU)用于腹腔神经节的损毁治疗胰腺癌疼痛,观察疗效并探讨其应用的安全性和有效性。方法:将26例病人随意分成A、B两组,A组16例为单纯行局部肿块HIFU治疗,B组10例用HIFU局部肿块治疗+腹腔神经节损毁。结果:26例患者在HIFU治疗中和治疗后均未出现任何副反应和并发症,A组治疗后早期(15天内)疼痛缓解率为13%,20天后疼痛与治疗前无明显差异。B组治疗后即刻疼痛明显减轻,治疗后3~5天疼痛缓解率为83.2%,食欲增加,精神状态明显好转,A、B两组间有统计学意义(Р〈0.05)。结论:高强度聚焦超声损毁腹腔神经节对晚期胰腺癌有良好的止痛作用,是一种安全、有效、无创治疗晚期胰腺癌疼痛的新方法。
Objective: To study the treatment of pancreatic cancer pain and its efficacy,efficiency and safety via damaging celiac ganglion using high-intensity focused ultrasound(HIFU).Methods: 26 patients were studied and were randomly divided into two experimental groups.Group A consisted of 16 patients provided with HIFU treatment on localized tumors only.The other 10 patients formed group B were given both HIFU treatment on localized tumors as well as damage on celiac ganglia.Results: All the 26 patients exhibited no side reaction or complication during and after HIFU treatment.Group A has a odynolysis rate equaling to 13% in the early post-treatment phase(within 15 days) and exhibited no obvious difference in painfulness compared with pre-treatment phase at 20 days after treatment.On the other hand,group B has a considerable abatement in painfulness immediately after treatment and its odynolysis rate of was 83.2% 3 to 5 days later.In addition,patients in group B have posed obvious improvements in appetite and mental state.Group A and B have statistical significance(P0.05).Conclusion: Damage on celiac ganglia using HIFU has a good analgesic effect in alleviating pain caused by terminal pancreatic cancer.HIFU is a new and non-intrusive method of treating pain caused by terminal pancreatic cancer with reliable efficacy and safety.
