                    背景:急性胰腺炎(AP)动物模型存在小肠细菌过度生长(SIBO),肠内营养(EN)可降低重症急性胰腺炎(SAP)患者胰腺继发感染的风险.目的:评估EN对SAP患者SIBO的影响.方法:13例CT严重度指数(CTSI)≥3分的AP患者随机分为EN组(7例)和胃肠外营养(PN)组(6例),所有患者均置入鼻空肠管,分别于第1 d、3 d、7 d行葡萄糖氢呼气试验(GHBT),计算患者每次GHBT的总产氢量.EN组患者于首次GHBT后即开始行EN,PN组患者于第二次GHBT后改行EN.结果:两组患者首次GHBT的总产氢量差异无统计学意义(P〉0.05),EN组第一、二次GHBT总产氢量下降值显著大于PN组(33.0对4.0,P〈0.05);两组间第一、三次GHBT总产氢量下降值相当,差异无统计学意义(P〉0.05).结论:EN可能对SAP患者的SIBO有抑制作用.
                
                Background: Small intestinal bacterial overgrowth (SIBO) occurs in animal models of acute pancreatitis (AP), and enteral nutrition (EN) can reduce the risk of secondary infection in severe acute pancreatitis (SAP) patients. Aims: To evaluate the effect of EN on SIBO in SAP patients. Methods: Thirteen AP patients with CT severity index (CTSI) I〉 3 were randomly allocated into EN group (7 cases) and parenteral nutrition (PN) group (6 cases). Naso-jejunum tube was placed in all the patients. Glucose hydrogen breath test (GHBT) was performed on 1st, 3rd and 7th day, respectively and total hydrogen production was calculated. Patients in EN group received EN after 1st GHBT, while patients in PN group started EN after 2nd GHBT. Results: There was no significant difference in total hydrogen production of 1st GHBT between the two groups (P〉0.05). The decrease of total hydrogen production from 1st to 2nd GHBT in EN group was significantly higher than that in PN group (33.0 vs. 4.0, P〈0.05). However, there was no significant difference of the decrease from 1st to 3rd GHBT between the two groups (P〉0.05). Conclusions: EN might inhibit SIBO in SAP patients.
