摘要
目的研究北京新形势下原有突发和重大传染病临床应答模式的现状及所遭遇的挑战。方法应用问卷调查,通过集合传染病、呼吸、急诊、医疗管理等相关领域专家和工作者的意见,征询对北京目前传染病应答模式中存在的问题和看法。结果共发放问卷81份,收回问卷78份,有效问卷76份;目前筛查门诊的作用没有完全发挥,尤其是发热门诊的作用受限;筛查门诊应设立观察病床,但不设住院病床,感染科设立住院病床是必要的;在目前政府扶持少的情况下,常年设置发热门诊是对人力、物力的资源浪费,可以采取肠道门诊分季节和流行趋势模式而建。结论突发和重大传染病防治的临床模式仍存在一定问题,发热门诊对传染病及时发现、报告、诊治等方面不够灵敏,效率不够高,政府及医疗部门应提供资金支持,固定医务人员,这样既有利于人才的发展,也有利于专业的发展。
OBJECTIVE To study present situations and challenges of clinical management modes for sudden events and severely infectious diseases.METHODS The opinions and suggestions about clinical management modes were collected from experts in the fields of infectious diseases,respiratory diseases,emergency and medical management etc by questionnaires.RESULTS From 81 questionnaires,78 were returned and 76 were valid.The results demonstrated that the functions of flu-screening clinics had not worked well,and especially fever-screening clinics only exerted limited effects.The sick beds for observation but not hospitalization should be setted in flu-screening clinics,and hospitalization beds should be setted in department of infectious diseases.It was a waste of human and material resources to set fever-screening clinics all year long with little supports by the government at present,but we suggested setting intestinal clinic according to epidemic trends of infectious diseases.CONCLUSIONS There are still some problems in the clinical management modes of sudden events and severely infectious diseases at present.The infectious diseases in fever-screening clinics could not be found effectively,reported in time,and not diagnosed or treated.The government and health departments should supply enough funds and medical staff to benefit the growth of talents.
