摘要
目的探讨特发性血小板减少性紫癜(ITP)患儿血清B细胞激活因子(BAFF)水平变化的意义。方法采用双抗体夹心ELISA法,对2008年4-11月初次在本院住院的42例ITP患儿(试验组)治疗前的血清BAFF水平进行检测,并进行血小板计数(BPC);同时检测40例拟择期手术患儿(对照组)血清BAFF水平。比较试验组治疗前后与对照组血清BAFF水平的差异,应用Pearson直线相关分析法分析试验组患儿治疗前后血清BAFF水平与血小板的相关性。结果ITP患儿治疗前BAFF水平为(0.943 3±0.583 5)μg.L-1,治疗后BAFF水平为(0.454 5±0.216 0)μg.L-1,对照组BAFF水平为(0.538 9±0.234 7)μg.L-1。ITP患儿治疗前血清BAFF水平显著高于对照组和ITP患儿治疗后(Pa<0.01),治疗后BAFF的表达较治疗前明显降低,但与对照组比较差异无统计学意义(P>0.05)。相关分析显示,外周BPC与血清BAFF水平呈负相关(r=-0.305,P<0.01)。结论增高的BAFF可能参与ITP的发病机制。
Objective To explore the significance of the changes of serum B cell activating factor（BAFF） in children with idiopathic thrombocytopenic purpura（ITP）.Methods The concentrations of BAFF were detected by ELISA in 42 children with ITP（test group） before and after treatment in the Children′s Hospital Affiliated to Chongqing Medical University from Apr.to Nov.2008,the blood platelet count（BPC） was detected also.And 40 children who were operated selectively were selected as control group.The variation of concentrations of BAFF were analyzed among children with ITP before,after treatment and control group.Moreover,the relationship between serum BAFF expression and BPC were analyzed by the Pearson test.Results The concentrations of serum BAFF were higher in children with ITP before treatment compared with that in control group [（0.943 3±0.583 5） μg·L^-1 vs（0.538 9±0.234 7） μg·L^-1,P〈0.01] and children with ITP post-treatment [（0.943 3±0.583 5） μg·L^-1 vs（0.454 5±0.216 0） μg·L^-1,P〈0.01],but there were no significant difference between after treatment and control group（P〉0.05）.There was negative correlation between serum BAFF expression and BPC（r=-0.305,P〈0.01）.Conclusion The high concentrations of BAFF may be involved in pathophysiology of ITP.
