摘要
目的:观察酒石酸布托啡诺鼻喷剂的安全性和有效性。方法:2008年7月～2008年12月,在北京6家医院开展了多中心临床观察,共252例癌痛患者入组。其中男性92例,女性160例;年龄22～86岁,中位年龄58.98岁。均为有明确病理诊断的晚期癌症患者,伴中、重度的癌性疼痛(24h平均疼痛强度评分:NRs评分≥4分),停用其他阿片类镇痛药物3天以上再使用酒石酸布托啡诺鼻喷剂治疗癌痛;初始剂量为每次1喷(相当于1mg),3～4次/日,观察患者疼痛缓解和生活质量改善情况,记录药物不良反应。结果:酒石酸布托啡诺鼻喷剂治疗中、重度疼痛,用药起效时间23.3±5.7min,疗效持续时间237.5±48.8min。完全缓解32例,占12.7%;明显缓解120例,占47.6%;中度缓解56例,占22.2%;轻度缓解36例,占14.3%;无缓解8例,占3.2%;有效率为82.5%,显效率为60.3%。对患者用药前后生活质量进行统计,76.2%患者治疗后有较明显改善。出现不良反应的患者占28.6%,主要症状为头晕、恶心、嗜睡等,程度均较轻。结论:酒石酸布托啡诺鼻喷荆治疗癌痛,疗效肯定,不良反应轻微。
Objective： To evaluate the safety and efficacy of transnasal butorphanol in cancer pain management. Methods： A total of 252 patients with cancer pain from six hospitals in Beijing were observed （92 males and 160 females, aged 22-86 years with an average of 58.98）. All of the patients in this study had a clear pathological diagnosis with moderate to severe cancer pain （a 24-hour average pain intensity score： NRS score ≥ 4 points）. Transnasal butorphanol was used to treat pain with an initial dose of one spray （equivalent to 1 mg）, 3-4 times a day. The efficacy and adverse reactions were observed. Results： All of the 252 patients were treated with transnasal butorphanol. Onset of analgesia was 23.3±5.7 minutes, and the duration of effect was 237.5 ± 48.8 minutes. Thirty-two cases （12.7%） had complete remission; 120 cases （47.6%） had apparent remission; 56 cases （22.2%） had moderate remission; and 36 cases （14.3%） had mild remission. The QOL of 76.2% patients was markedly improved after treatment. Only 28.6% cases suffered from mild adverse reactions including dizziness, nausea, and lethargy. Conclusion： The therapeutic effect of transnasal butorphanol was stable and the adverse effects were mild.
