晶状体囊外摘出术中低分子量肝素前房灌注对房水bFGF和TGF-β2含量的影响及意义 被引量:2

摘要 目的观察房水碱性成纤维细胞生长因子(basic fibroblast growth factor,bFGF)和活性转化生长因子-β2(transforming growth factor-β2,TGF-β2)含量的变化,探讨低分子量肝素(low molecular weight heparin,LMWH)前房灌注对后发性白内障形成的影响机制。方法选取84只兔84眼随机平均分为模型组和实验组(每组各42眼),行单眼透明晶状体囊外摘出术。实验组术中单次应用50000 IU·L-1 LMWH进行前房灌注。分别于术后1d、3d、6d、15d、30d、45d、60d每组各取6只兔6眼,分别测定房水bFGF和活性TGF-β2含量;同时于术后1d、30d、45d、60d分别称取晶状体囊袋湿质量。另选取3只兔6眼作为正常对照组,测定房水bFGF和TGF-β2含量的基线水平。结果晶状体囊外摘出术后模型组和实验组囊袋湿质量均逐渐增加,但LMWH促进了湿质量增加速度,至术后60d时实验组囊袋湿质量为(123.66±17.83)mg,高于模型组的(97.25±15.99)mg(P<0.05)。模型组术后房水bFGF浓度增加了8~15倍,而实验组仅增加了3倍。模型组房水活性TGF-β2浓度在术后30d开始高于基线值(P<0.05),而实验组活性TGF-β2浓度始终与基线值无统计学差异(P>0.05)。模型组术后房水bFGF和活性TGF-β2的含量分别于术后30d和45d恢复至基线水平,而实验组则分别于术后3d和1d恢复至基线水平。结论晶状体囊外摘出术中单次应用实验剂量的LMWH促进后发性白内障形成,可显著抑制术后房水中bFGF和活性TGF-β2表达强度和持续时间。推测房水中这2种细胞因子的含量变化共同参与了后发性白内障的形成。 Objective To measure the levels of basic fibroblast growth factor (bFGF) and transforming growth factor-β2 (TGF-β2) in aqueous humor (AqH), and to discuss the mechanism of low molecular weight heparin (LMWH) on after-cataract formation in a rabbit model. Methods Extracapsular lens extraction was performed on unilateral eye in 84 rabbits (84 eyes) which were divided into two groups including model group and experimental group, 42 eyes in each group. Experimental group was treated with 50 000 IU · L^-1 LMWH into anterior chamber. The concentrations of bFGF and active TGF-β2 in AqH were measured at 1 day,3 days,6 days,15 days,30 days,45 days, and 60 days after operation, respectively. The wet mass of capsular bag were obtained 1 day,30 days,45 days, and 60 days after operation, respectively. The concentrations of bFGF and active TGF-β2 in AqH were measured in three normal rabbits as the baseline. Results The wet mass of capsular bag was gradually increased after opera- tion in both groups, and LMWH could speed up this increasing. On the 60th day after operation,the wet mass of capsular bag in experimental group was ( 123.66 ± 17. 83) mg, showing a significant difference compared with (97.25 ± 15.99) mg,the value in model group(P 〈 0.05 ). The bFGF concentration in AqH increased 8- to 15-fold after surgery in model group, but only 3-fold in experimental group. The AqH concentration of active TGF-β2 began increasing at 30 days after surgery in model group (P 〈 0.05 ), but kept on baseline in experimental group(P 〉 0.05 ). The levels of bFGF and active TGF-β2 in AqH returned to baseline at 30 days and 45 days after surgery in model grouo, and at 3 daysand 1 day after surgery in experimental group,respectively. Conclusions Single application of LMWH at the tested dose can promote after-cataract formation, and inhibit the extent and duration of the expressions of bFGF and active TGF-β2 in AqH after extracapsular lens extraction. It is suggested that these two cytokines in aqueous humor participate in after-cataract formation.
