摘要
大骨节病(KBD)病因至今未明。本文对笔者等近年来撰写的KBD有关病毒病因的多项研究报告做了综合性论证。就笔者研究所见KBD有三类细胞坏死:(剖检材料上的)软骨细胞B型凝固性坏死和骨髓血细胞坏死及(KBD病儿的)外周血白细胞坏死;三者坏死细胞的核内都有嗜酸性包涵体,且三者坏死的形态改变及其演化过程相同又都有KBD病理特异性。据此笔者认为,三者细胞坏死可能是由同一病毒感染引起的。在KBD病儿的坏死白细胞核内找出病毒核衣壳的结果,已使白细胞坏死是由某特异病毒感染引起的这一假设得到验证。作为关键病变,KBD软骨细胞B型凝固性坏死可能也是由引起白细胞坏死的该病毒的同一病毒感染引起的,即该病毒可能就是KBD的病因。后一假设已得到体外实验结果的强有力支持:实验中该病毒已诱发培养软骨细胞出现与人类KBD软骨细胞B型凝固性坏死相一致的特异病变。有关KBD病毒病因假设的结论性评价,有待后继更多工作来检验。
The cause of Kashin - Beck disease （KBD） is still uncertain. A comprehensive demonstration on the most recent researches concerning viral cause of KBD published in my group is carried out in this paper. Based on the author＇s research, there are three kinds of cell necroses in KBD： the type B coagulation necrosis of chondrocytes and the necrosis of bone marrow blood cells in autopsy samples with KBD and the necrosis of white blood cells in peripheral blood of children with KBD. They all have acidophilic inclusion bodies in their nuclei, show the uniformly morphologic changes and developing processes in their cells, and all these are peculiar to KBD. Therefore, the author believes that all these three kinds of the necroses might be caused by the same virus infection. The hypothesis that the cause of necrosis of white blood cells might be attributed to the specific virus infection has been verified through the result that some viral nucleocapsids present in the nuclei of the necrotic white blood cells from KBD children. As a key necrosis, the type B coagulation necrosis of the chondrocytes in KBD corpses might also be induced by the same virus that induced the necrosis of white blood cells. In other words, the virus might be the cause of KBD. This hypothesis is strongly supported by the experiment result that the virus in vitro has caused the uniform necrosis of cultural chondrocytes with the type B coagulation necrosis of chondrocytes in KBD corpses. Further, more studies are called on to confirm the hypothesis about the virus cause of KBD.
