骨髓干细胞移植治疗2型糖尿病合并下肢血管病变临床观察 被引量:7

Clinical observation of autologus bone marrow stem cell transplantation in treatment of type 2 diabetes patients combined with lower extremities vascular lesions

摘要 目的探讨2型糖尿病合并严重下肢血管病变临床特点及自体骨髓干细胞移植治疗后的转归及临床意义。方法3例2型糖尿病合并严重下肢血管病变伴久治不愈糖尿病性足坏疽患者,其中2例为男性,1例为女性,糖尿病病程8-14年,在传统治疗(包括内科保守治疗及血管介入治疗)效果不理想的情况下,行自体骨髓干细胞移植治疗。结果3例糖尿病患者均存在严重的糖尿病足坏疽,下肢血管超声显示糖尿病下肢动脉有不同程度的斑块、狭窄与血栓形成,病变累及下肢股动脉、月国动脉等。在自体骨髓干细胞移植治疗前后,所有患者均行胰岛素控制血糖,抗感染、活血化淤改善微循环,其中1例实施了下肢血管造影检查及介入治疗,经自体骨髓干细胞移植治疗1个月后,所有患者足部临床表现得到一定的改善,治疗过程中无不良反应。结论自体骨髓干细胞移植对于糖尿病合并严重下肢血管病变,尤其是久治不愈的糖尿病足坏疽是一种可选择的治疗方式。 Objective To investigate the characteristics of type 2 diabetes combined with lower extremities vascular lesions,and to explore the treatment effect of autologus bone marrow stem cell transplantation. Methods Three diabetic patients with lower extremities vascular lesions and diabetic foot gangrene were treated with traditional therapy including internal medicine and interventional therapy with less effect, and then were given autologus bone marrow stun cell transplantation. All patients were performed ultrasonic Doppler examination. Before transplantation, the past history, blood lipids, BMI and blood pressure were determined, and one patient received angiography examination and interventional therapy. Results Three patients suffered from serious lower extremities vascular lesions, with rest pain and intermittent claudication. After autologus bone marrow stem cell transplantation, lower extremities vascular lesions were improved, and rest pain and intermittent claudication were relieved. Conclusion Lower extremities vascular lesions and diabetic foot gangrene are serious complications in diabetic patients. Autologus bone marrow stem cell transplantation is an effective way to ameliorate foot gangrene and reduce disabling.
