摘要
目的:探讨多层螺旋CT血管造影(MSCTA)及三维重建对原发性肝癌重复介入治疗的指导价值。方法:对40例原发性肝癌患者,再次进行介入治疗时先行MSCTA检查,并用最大密度投影(MIP)、容积成像(VR)、多平面重建(MPR)技术对血管进行三维重建。结果:40例原发性肝癌患者共进行MSCTA检查及三维重建76次,共发现肝动脉解剖变异8例,栓塞治疗后肿瘤侧支循环形成28例,肝动脉闭塞2例。MSCTA显示腹腔动脉及肝动脉分支的三维结构以及介入治疗后侧支循环形成、来源方面优于DSA。结论:MSCTA及三维重建能直观、清晰的显示肿瘤病灶的供血血管立体解剖图像,对再次介入治疗制定手术方案具有重要指导意义。
Objective:To assess the instruction value of the MSCrA and three-dimensional reconstruction to the repeated intervention treatment for primary liver cancer. Methods: MSCTA were performed in 40 cases of primary liver cancer before and after interventional treatment. Three-dimensional reconstruction were done in blood vessels using the technique of MIP VR and MPR. Results: In total, 76 times of MSCTA and 3D reconstruction were performed in these 40 patients, of which 8 cases were found anatomical abnormality of hepatic arteries, 28 cases were found to have formed collateral branches after embolism, and 2 cases were found to appear the arterial atresia. MSCTA was superior to DSA in showing 3D reconstruction of celiac artery and hepatic artery branches as well as the collateral branch formarion and derivation after embolism. Conclusion: MSCTA and 3D reconstruction can intuitively and clearly demonstrate the anatomy image of the feeding arteries of the tumor, which are of great value in the guidance of repeated interventional treatment of primary liver cancer.
