摘要
目的:探讨新生儿窒息后血肌钙蛋白T(cTnT)及血心肌酶谱水平测定对判断心肌损害的临床价值。方法:围产期窒息新生儿100例,其中重度窒息37例、轻度窒息63例,无窒息新生儿80例,均于生后20min采血检测cTnT、血清谷草转氨酶(AST)、乳酸脱氢酶(LDH)、肌酸磷酸激酶(CK)及肌酸磷酸激酶同工酶(CK- MB),并对其进行对比分析。结果:窒息新生儿的血cTnT、AST、LDH、CK、CK-MB水平均明显高于非窒息新生儿(P<0.05,P<0.01),且窒息程度越重,各指标的水平越高(P<0.01)。结论:窒息新生儿的血cTnT与心肌酶谱明显改变,且与窒息程度相关,可作为判断新生儿缺血缺氧性心肌损害的特异性指标,对新生儿窒息心肌损害的早期诊断与干预有重要临床意义。
Objective:To investigate the clinical value of detecting troponin t (cTnT) and myocardium enzymogram in blood of newborn with asphyxia to judge myocardial damage.Methods: 100 newborns with asphyxia that included 63 mild and 37 severe asphyctic newborns were chosen in this study,and 80 non-asphyxia newborns were taken as control at the same time. The blood of all newborns were collected 20-min after birth;and the cTnT,AST,LDH,CK and CK-MB in blood were dectected respectively.Results:The level of cTnT and myocardium enzymogram in blood of asphyctic newborns were obviously higher than that of non-asphyxia newborns (P 〈 0.05,P 〈 0.01).Moreover,each index increased with degree of asphyxia (P 〈 0.01). Conclusions:cTnT and myocardial zymogram of asphyctic newborns obviously change,and the changes has relations with the degree of asphyxia.So they can be specificity indexes to judge the myocardial damage induced by ischemia and anoxia;and they play important role in early diagnosis and intervention for myocardial damage of asphyctic newborns.
