消化系肿瘤浸润淋巴细胞不同输注途径对免疫功能的影响

The Effect of Autoinfusion of Alimentary Tumor Infiltrating Lymphocytes by Different Ways on Immunologic Function

摘要 从肿瘤组织中分离出TIL,经rIT-2培养扩增后进行自体回输,输注途径包括:(1)外周静脉;(2)局部(肿瘤供血动脉或腹腔);(3)静脉加局部。TIL输注总量为1.5×10^9以上。治疗前后抽取外周静脉血检测T细胞亚群和NK细胞活性。18例患者(胃癌9例、大肠癌5例、肝癌4例)经TIL治疗后,88.9%的患者NK活性增强,77.7% ̄94.4%的患者CD3+、CD4+和CD8+细胞比例升高。 TILs which were isolated from tumor tissue, and cultured with rIL - 2 for expansion were autoinfused by way of (1) peripheral vein; (2)local (blood supplying artery for tumor or peritoneal cavity); (3)vein plus local, at a total amount of more than 1.5×109. Before and after treatment, peripheral blood was taken for the detection of T subgroup cells and NK activity. 18 cases (gastric Ca 9, colonic Ca 5, liver Ca 4) were treated with TIL, of whom NK activity was enhanced in 88.9%,CD3+ , CD4 + , and D8+ subgroup were elevated in 77.7%-94.4%, showing different ways of infusion had a different effect on immunologic function. It has been suggested that TIL in vivo by way of venous infusion can secreat cytokine to enhance the immunologic function. However it is the best way to combine local infusion with venous one for enhancement of immunologic function as well as concentration of TIL in tumor tissue.
