外周血基质金属蛋白酶9、高敏C反应蛋白、白细胞计数与2型糖尿病及其大血管病变的相关研究 被引量:10

摘要 目的评价血清基质金属蛋白酶9(MMP-9)、高敏C反应蛋白(hsCRP)、外周血白细胞(WBC)计数与2型糖尿病(T2DM)及其大血管病变的关系。方法分别用酶联免疫吸附(ELISA)法和高敏免疫透射比浊法测定40例健康对照者和100例T2DM患者(其中单纯糖尿病者50例,合并大血管病变者50例)血清MMP-9、hsCRP水平,同时测定外周血WBC计数和其他临床指标,分析各指标之间的相关性及与大血管病变的关系。结果与健康对照者相比,糖尿病患者外周血MMP-9、hsCRP、WBC明显升高(P<0.017),合并大血管病变者明显高于无并发症者(P<0.017)。三者呈统计学意义的正相关,以MMP-9与hsCRP的相关性更强(r=0.738)。3指标均与糖化血红蛋白(HbA1c)、胰岛素抵抗稳态模型指数(HOMA-IR)、低密度脂蛋白胆固醇(LDL-C)呈正相关。MMP-9、SBP是T2DM大血管病变的独立危险因素,高密度脂蛋白胆固醇是保护因素。结论外周血MMP-9、hsCRP、WBC 3项指标的联合测定更有助于临床上认识DM大血管病变、急性心脑血管事件发生的危险性,从而早期积极采取有效的干预措施。 Objective To evaluate the relationship of matrix metalloproteinase-9 (MMP-9), high sensitivity C- reactive protein(hsCRP),white blood ceII(WBC) and type 2 diabetes mellitus,also its macroangiopathy. Methods Serum MMP-9 and hsCRP were determined by enzyme linked immunosorbent assay(ELISA) and high-sensitivity immunoturbidimetric method in 40 normal controls and 100 cases of type 2 diabetes(including 50 cases of type 2 diabetic macroangiopathy and 50 cases of type 2 diabetes mellitus without complications). WBC and other clinical parameters were determined to analyse their relationship with macroangiopathy. Results In type 2 diabetic patients wth macroangiopathy,serum MMP-9, hsCRP and WBC were higher remarkedly than those in type 2 diabetes without complications. The above three items were significantly correlated positively with HbA1c, HOMA-IR and low-density lipoprotein cholesterol. MMP-9, SBP were independent risk factors of T2DM macroangiopathy, and the high-density lipoprotein cholesterol was a protective factor. Conclusion Combined application of the three parameters, MMP-9, hsCRP and WBC contribute to realise the risk of T2DM macroangiopathy and acute cardiovascular events for effective measures early.
