丙泊酚靶控输注用于颈丛神经阻滞下甲状腺手术镇静的可行性 被引量:16

摘要 目的探讨丙泊酚靶控输注(TCI)用于颈丛神经阻滞下甲状腺手术镇静的可行性及血浆靶浓度。方法随机选择甲状腺手术患者30例ASAⅠ~Ⅱ级,颈丛神经阻滞完善后,采用Dipri—fusor TCI系统,以0.4μg/ml为初始靶浓度(血浆靶浓度)开始输注丙泊酚,根据改良镇静/警醒评分(OAA/S评分)适当调整浓度(靶浓度每3分钟递增0.1μg/ml),直至镇静深度稳定于OAA/S评分为3分,记录OAA/S镇静稳定时的靶浓度,观察指标以TCI前MAP、HR、RR、SpO2为各自基础值,观察记录OAA/S评分为5,4,3,2分时丙泊酚靶浓度值,同时记录血液动力学参数。结果OAA/S镇静稳定为3分时的血浆靶浓度为(1.68±0.21)μg/ml,术中各时点MAP、HR、RR、SpO2差异无显著意义。结论丙泊酚TCI用于颈丛神经阻滞下甲状腺手术患者镇静的可控性强,血液动力学稳定,镇静深度适宜,可有效抑制患者术中的应激反应。
