2型糖尿病合并高血压患者大血管病变的炎性机制 被引量:9

The inflammation mechanism between macrovascular disease and type 2 diabetes with essential hypertension

摘要 目的探讨2型糖尿病合并高血压时慢性炎性因子与大血管病变的关系。方法131例2型糖尿病患者分为4组,比较各组糖尿病并发高血压和大血管病变时的细胞因子水平。结果正常对照与4组糖尿病亚组相比,CRP、IL-2、IL-6、IL-8、TNF-α、PAI-1水平差异均有统计学意义。糖尿病并大血管病组的CRP、IL-6、TNF-α、PAI-1水平显著高于糖尿病无大血管病变组(P<0.05);糖尿病高血压合并大血管病变组IL-6、TNF-α水平显著高于糖尿病合并大血管病变组(P<0.05)。IL-6、TNF-α与BM I、WHR、FFA、TC、FINS、UA均呈正相关,BM I、FINS作为独立因素影响IL-6、TNF-α的水平。结论IL-6、TNF-α可能在2型糖尿病大血管病变尤其同时合并高血压时发挥了一定的炎性作用。 Objective To study the relationship between macro vascular disease and chronic inflammatory factors in type 2 diabetes with essential hypertension. Methods 131 diabetic patients( DM2 group) , which were further subdivided into 4 subgroups according to whether the patients had hypertension or macro vascular complications. Results The plasma concentrations of CRP, IL-2, IL-6, IL-8, TNF-α, PAI-1 in four diabetic subgroups were significantly higher compared with the controls( P 〈 0. 01 ) ; CRP, IL-6, TNF-α, PAI-1 levels in diabetic patients with macro vaseular disease were higher than those of diabetic patients without vascular disease( P 〈 0. 05 ) ; IL-6 and TNF-αlevels in diabetie patients with hypertension and macro vascular disease were signifieantly higher than those of diabetic patients with vascular disease only( P 〈0. 05). There was a positive correlation between the PAI-1, IL-6, TNF-αlevel and BMI, WHR, FFA, TC, FINS, UA levels in diabetic patients. BMI, WHR were equal predictors of plasma PAI-1 , IL-6, and TNF-α. Conclusions IL-6 and TNF- αwere involved during the development of macro vascular disease in diabetes , especially with essential hypertension.
