中心静脉导管相关性真菌败血症的临床分析 被引量:4

摘要 目的探讨真菌性中心静脉导管(CVC)相关性败血症(CRS)的临床特点、发病机制与防治措施。方法回顾性分析34例真菌性CRS患者的基础疾病、易患因素、导管真菌培养结果、治疗措施。结果所有患者均在严重基础疾病和应用广谱抗生素的基础上发病,CVC留置时间≥7d,4例患者两次留置CVC。分离得到38株真菌,白色念珠菌占68·4%(26/38)。6例患者拔管36h后体温降至正常;另28例患者静脉给予氟康唑,其中11例换用或加用两性霉素B。8例(23·5%,8/34)患者继发多器官功能衰竭而死亡。结论减少发病诱因,及早确诊是防治真菌性CRS的关键。 Objective To explore the clinical characteristics, pathogenesis, prevention and treatment of catheter related septicemia(CRS) caused by fungi. Methods The data (such as basic diseases, predisposing factor, rcsubs of culture and treatment) of 34 cases of confirmed fungal CRS were analyzed retrospectively. Results All 34 cases were infected base on .severe elementary diseases and using broad spectrum antibiotic. Central venous catheters (CVC) were kept more than 7 days, 4 cases had been placed CVC for twice. 38 strains of fungus were separated from patients and 26 strains were candida albicans (68.4%). The temperature of 6 patients decreased to normal 36 hours after extubation. 28 cases were used with fluconazole, and 11 of them associated with amphotercin B. 8 cases died of multlpe organ failure. Conclusions Reducing predisposing factors and earlier diagnosis were the key measure to prevent and cure fungous CRS.
