摘要
《医林改错》是清代医家王清任的著作。该书的主要学术特点及对临床指导意义在于:1)业医诊病,当先明脏腑;2)阐发气血理论,创立脑髓学说;3)理论联系实际,创立多首名方。学习该书,首先要学习王清任师古不泥,学有创意,从实践中寻求真知的精神;其次,要学习王清任善于在临证观察中发现问题和解决问题的方法;再者,要认真领悟王清任最为擅长的活血化瘀方药的立法和组方思路。笔者以脑窍通方、清脑通络方、通脉舒络汤、血府逐瘀汤、膈下逐瘀汤、少腹逐瘀汤和身痛逐瘀汤为例略述活血化瘀方药在临床的应用。
Yilin C, aicuo（Correction on the Errors of Medical Works） is a work of WANG Qing-ren in Qing dynasty. The main academic characteristic and clinical guiding effect consist in three aspects： 1） putting the differentiation of ZANG-FU Organ on the first place in diagnosis of disease, 2） elucidating the Qi-Xue（vital energy and blood） theory and creating a theory of brainspinal cord, 3） associating the theory and practice to set up several famous prescriptions. In the study of the book we should learn the spirit of seeking after the truth from the practice and learning from the teachings of the ancient medical master without adhering to them at first. Secondly, learn the methods of discovering and resolving the problem from the clinical observation. Finally, earnestly understand the working out a principle of therapy and train of thought of the drugs and prescriptions of Huoxu Huayu （activating blood circulation and removing stasis）, which is the forte of WANG Qing-ren. Taking Naocliao-tong（dredging brain-orifice） prescription, Qingnao Tongluo（clearing brain and dredging collateral） prescription, Tongmai Shuluo（dredging the meridian and dispersing collateral） prescription, Xuefu Zhuyu （removing blood stasis in the house of blood） decoction, Gexia Zhuyu（removing blood stasis below diaphragm） decoction, Shaofu Zhuyu（removing blood stasis in lower abdomen） decoction and Shentong Zhuyu（removing blood stasis for treating pantalgia） decoction as the examples the clinical application of the drugs and prescriptions of Huoxue Huayu has been described briefly.

张学文（1935-），男，陕西汉中人，曾任陕西中医学院内科教研室主任、医疗系主任、院长，曾任国家中医药管理局重大科技评审委员，陕西省科技进步奖评审委员，陕西省第八届政协委员等，现任陕西中医学院教授、主任医师、学院专家委员会主任委员，北京中医药大学兼职博士生导师，咸阳市中医疑难病研究所所长、咸阳心脑血管病医院名誉院长，为中华中医药学会内科分会顾问，陕西省中医药学会顾问，《中医杂志》、《中医药学报》、《中医急诊》等杂志编委，并任美国中国医学研究院顾问，日本国群马中医研究会顾问，1990年被国家二部一局确定为首批全国500名需要继承学术经验名老中医之一，1991年起为享受国务院特殊津贴的专家，1991年被评为陕西省有突出贡献专家，曾先后应邀去日本、美国、东南亚、欧洲、澳洲、非洲多个国家及港澳台地区进行学术交流，从医50余年，执教40余载，自1978年至今，已培养温病学、中医脑病学专业硕士研究生72名，博士研究生4名，国家级师承弟子4名，国外弟子5名，在中医急诊、中医脑病、温病学、疑难病、活血化瘀等诸多领域均有所研究，对“毒瘀交夹”、“水瘀交夹”、“痰瘀交夹”、“气瘀交夹”、“颅脑水瘀”等病机理论的认识颇多创新，自成体系，先后在全国省级以上学术刊物发表论文60余篇，出版学术专著10余部，获国家、部、省厅、局级科技成果奖20余项。
