前庭小管周围骨质发育与梅尼埃病关系的螺旋CT影像学研究 被引量:1

Image study on the relationship between the development of bone around vestibul araqueduct and the lesion of meniere’s disease by spiral CT scanning

摘要 目的通过观察梅尼埃病(Meniere’s Disease,MD)患者前庭小管(VestibularAqueduct,VA)及其周围骨质发育情况,探讨MD的病理解剖学基础。方法实验分为两组:正常耳组和MD组。对每个研究对象进行颞骨螺旋CT扫描。在围绕外半规管的横向轴位二维重建图像上,选择显示外半规管及后半规管后外侧断面最清晰的层面,直接测量:1后半规管断面中心到颞骨岩部后面的最短距离(PP值);2前庭到颞骨岩部后面的最短距离(VP值);3后半规管至水平半规管的最短距离(PL值),并在所有显示迷路的层面上观察迷路后有无气房。结果MD组PP值为2.16±1.02mm,正常耳组PP值为3.41±1.51mm,其差异具有非常显著性意义(t检验,P<0.01);MD组VP值为4.28±1.06mm,正常组为5.87±1.21mm,其差异具有显著性意义(t检验,P<0.01);MD组PL值为2.72±1.12mm,正常组为3.14±1.04mm,两者差异无显著性意义(t检验,P>0.05);MD组有迷路后气化者占13.51%,正常耳组占73.33%,其差异具有非常显著性意义(t检验,P<0.01)。结论MD患者VA周围骨质发育障碍,岩骨迷路后气化不良。 Objective To explore the pathological anatomic basis underlying the pathogenesis of Menlere's disease (MD) through image study on bone development around vestibular aqueduct(VA) among cases with this lesion by spiral CT scanning. Methods The cases included in this study were divided into normal group and MD one. Every case in this study was give spiral CT scanning on temporal bone and 2D image reconstruction was performed in the transverse axial of lateral semicircular canal and posterior canal, retro-lateral section Then, direct measurement was made on the most clear images to determine the shortest distance from the center of posterior canal section to the posterior surface of petrous bone (PP value), the shortest distance from vestibule to posterior surface of pertous bone (VP value)and the minimal distance from posterior canal to lateral canal(PL value) ,with a determination on retro-labyr in thine air cells on all slices displaying labyrinthine. Results he mean PP value was 2.16±1.02mm, VP value was 4.28± 1.06mm and PL Value was 2.72±1.12mm in the cases of MD group all significantly shorter than those Of 3. 14±1.51mm,5. 87±1.21mm and 3.14±1.04mm respectively in the cases of normal group(P〈0. 01),other than that of PL value(P〉0.05). Moreover, there was a rate of 13.51% with retro-labyrinthine air cells in MD group and a rate of 73.33% with this pneumatic type of temporal bone in normal group, showing very significant diference between them. Conclusion Mal-development in the bone around VA should be true as an underlying factor of pathogenesis for cases with MD.
