新生猪缺氧缺血性脑病模型可逆缺血区的ADC值与HSP 70对照研究 被引量:11

摘要 目的:利用磁共振ADC值测量与HSP 70免疫组化检测技术,研究新生猪急性期重度缺氧缺血性脑病(HIE)模型脑部可逆缺血区的存在与否及其演变规律.方法:7日龄健康新生约克猪45只随机分为正常对照组(5只)和HIE组(40只).HIE组于缺氧缺血后24h内不同时间点采集ADC值与HSP 70资料.ADC值测量与HSP 70阳性细胞计数采用ADC值最低的区域至ADC值正常区域等距4分区法.结果:HIE组Ⅰ区ADC值下降的幅度最大,随时间呈先降后升的变化规律,HSP 70阳性细胞计数随时间未见明显变化,病变趋向坏死;Ⅱ区、Ⅲ区ADC值下降的幅度依次递减并随时间逐渐上升;Ⅳ区ADC值保持正常水平;Ⅱ~Ⅳ区HSP 70阳性细胞计数均随时间明显增加,病变趋向可逆.结论:HIE病理演变过程中的ADC值与HSP 70表达之间具有很好的一致性,ADC值变化具有鉴别HIE过程中脑组织坏死和可逆脑缺血损伤的能力. Objective:To study reversible cerebral ischemia regions of acute severe hypoxic ischemic encephalopathy (HIE) in piglet model by MRI ADC values and HSP 70. Methods:Seven days old piglets (n=45) were divided into 2 groups at random :group A (normal control group, n= 5 ) and group B (HIE group, n= 40, produced by bilateral common carotid artery occlusion and then inspiration of 4% oxygen about 1 hour). On ADC maps,4 regions of interest (ROD were set from the minimum ADC values point to the normal ADC values area, and named ROI Ⅰ , ROI Ⅱ , ROI Ⅲ , ROI PC in order. The data of ADC values and HSP 70 positive cells were acquired at the time point of 0,1,3,6,12,18 and 24h after formation of hypoxie isehemia. Results: In HIE group, the ADC values of ROI Ⅰ dropped first and then rose, and it dropped most seriously at 12~18h after formation of hypoxie isehemia. The one of ROI Ⅱ and ROI Ⅲ dropped less seriously when compared with ROI Ⅰ and rose gradually just after formation of hypoxie isehemia. The one of ROI IV was normal. The amount of HSP 70 positive cells in ROI Ⅰ was small and it did not increase with the time passing on. The amount of HSP 70 positive cells in ROI Ⅱ~Ⅳ was large and it increased obviously with the time passing on. Conclusion: Consistent with the HSP 70,the ADC values can make differentiation between reversible and irreversible cerebral ischemia regions in HIE.
