摘要
目的应用经颅多普勒超声(TCD)检测正常新生儿和缺氧缺血性脑病(HIE)患儿的脑血流动力学的变化,以评价其可靠性及应用价值。方法正常新生儿10例,轻、中、重度缺氧缺血性脑病患儿30例,分别行TCD检测,并分析各组间TCD结果的差异性。结果40例新生儿TCD频谱显影良好,TCD检测大脑中动脉的Vm、Vs、Vd、PI、RI与正常组新生儿比较,轻度组HIE无显著性差异(P>0.05);中度组有显著性差异(P<0.05);重度组有极显著性差异(P<0.01)。结论TCD能客观评价中、重度HIE患儿的脑血流动力学的变化,为HIE患儿早期诊断、干预选择及预后评估提供了有价值的信息。
Objective To find out the change of cerebral blood flow dynamics in newborns with hypoxic-ischemic encephalopathy(HIE) by transcranial Doppler(TCD), and investigate the reliability and the applicative value of TCD in the diagnosis of HIE. Methods Ten normal newborns and thirty newborns with low grade, medium, and severe HIE were randomly selected. The values of Vm, Vs, Vd, PI and RI of middle cerebral artery (MCA) in each subject were checked by TCD. The difference of TCD examination results of these groups was evaluated. Results A good spectrum of TCD can be acquired in each of the forty newborns. There was no difference in the values of Vm, Vs, Vd, PI and RI of MCA in the newborns with low grade HIE compared to the normal (P 〉 0.05). But there were significant differences in the values in the newborns with medium HIE( P 〈 0:05) and with high grade HIE( P 〈 0.01 ) compared to the normal. Conclusion The change of cerebral blood flow dynamics in newborns with medium and high grade HIE can be evaluated correctively and objectively by TCD. It is suggested that TCD is useful for early diagnosis, therapy and prognosis of newborns with HIE.
