摘要
目的探讨房水和血清中神经元特异性烯醇化酶(或称γ-烯醇化酶)同工酶(neuron-spe-cificenolase,NSE)测定在视网膜母细胞瘤中的诊断价值。方法应用醋酸纤维膜板快速电泳法分析23例(房水21例,血清23例)视网膜母细胞瘤(retinoblastoma,Rb)及25例(房水21例,血清25例)对照组房水和血清烯醇化酶同工酶,并用荧光箔层扫描仪测定其同工酶的相对活性及其所占百分比。结果Rb患眼房水全部发现αα、αγ、γγ3种烯醇化酶同工酶呈强阳性的荧光带,NSE阳性检出率为100%,NSE相对活性占3种烯醇化酶同工酶总相对活性的百分比为45%±9%;而对照组患眼房水,仅在2例外伤性前房积血的血性房水中发现αα及较弱αγ、γγ荧光带,在白内障、青光眼及4例Coats病患者中,未发现任何荧光带。23例Rb患者血清中,有18例发现α、αγ和/或γγ荧光带,NSE阳性检出率为78.2%;5例仅见αα带,其NSE相对活性强度及其占总相对活性百分比为36%±9%,均较房水明显。在对照组血清中,仅见αα带,NSE阴性率为100%。结论视网膜母细胞瘤房水和血清中NSE显增加,免疫电泳测定对视网膜母细胞瘤具有诊?
ObjectiveNeuron-specificenolase(NSE)orisoenzymescontainingγ-enolasearecon-sideredvaluableinthediagnosisoftumorsofneuroectodermalorigin.MethodWeusedrapidelec-trophoreticmethodonceluloseacetateplatetodeterminethepaternsofenolaseisoenzymesinthe21aqueoushumorand23serumspecimensfrom23patientswithretinoblastoma(Rb)and21aqueousand25serumspecimensfrom25controlcasestoevaluateNSEinthediagnosisofRb.Theassayalowedas-sessmentofalthreemajorisoenzymes(αα,αγandγγ),andNSErelativeactivityanditspercentageinthetotalrelativeactivityofthethreeenolaseisoenzymeswereassessedbymeansofafluorometer.Re-sultsAqueousfromalpatientswithRbcontainedαα,αγandγγisoenzymesandpresentedstrongpositive,thepositiverateofNSEbeing100%anditsrelativeactivityacountingfor45±9%oftheto-talrelativeactivityofthe3enolaseisoenzymes;noenolasewasdetectableinthecontrolaqueouswithcataract,glaucomaandCoats'sdiseases(4cases),butintwopatientswithtraumatichyphemaααandfaintαγandγγpresentedintheaqueous.Thecontrolserumcontainedonlyanααband,whiletheserafrompatientswithRbcontainedαα,αγandγγbandsin18of23specimens,thepositiveratebeing78.2%,andonlyααbandsintherest5specimens(21.8%).InthepatientswithRb,theNSErelativeac-tivityanditspercentageinthetotalrelativeactivityofthe3isoenzymesinserum(36±9%)weresig-nificantlylowerthanthatinaqueous.ConclusionTheamountsofNSEincreasesignificantlyinbothserumandaqueousfrompatientswithRb,andtheimmunoelectrophoreticassayfordeterminationofenolasepaternsisvaluableinthediagnosisanddiferentialdiagnosisofRb.Inaddition,themethodisrapid,simpleandrequiresonlyalitleamount(<5μl)ofsample.
