实验性颅高压中颅内容积代偿规律的探讨 被引量:1

摘要 目的 制成颅高压动物模型,探讨颅高压状态下颅内容积代偿及颅内顺应性的变化规律。方法 15条狗安置硬膜外球囊并注液制成颅高压模型。通过改变球囊内液体量来改变颅高压的程度和颅内容积。通过压力传感器持续记录脑室内压力(VICP)和腰椎管内压力(LICP)。结果 VICP和LICP的自然对数转化值与时间变化呈现负性线性相关;其中颅内容积增加<3 ml数据段的平均线性斜率绝对值明显大于颅内容积增加>3 ml数据段的平均线性斜率绝对值;颅压升高实验第一阶段中数据段的平均线性斜率绝对值明显大于第二阶段中数据段的平均线性斜率绝对值。结论 颅内容积增加后,颅内压下降趋势呈指数关系,其下降速度随着颅内容积的增加而减慢,3 ml的颅内容积增加量为颅内容积代偿的转折点,此时颅内顺应性明显下降。 Objective To study the regulation of intracranial volume compensation during intracranial hypertension. Methods Models of intracranial hypertension of 15 healthy mongrel dogs were created by placing the latex sacculus epidurally and injection of physiological saline. One ml physiological saline was injected into epidural sacculus every 10 min while the ventricle intracranial pressure (VICP) and intrathecal pressure of lumbar vzertebral canal (LICP) were recorded continuously. Results A negative linear relationship was found between the transformed natural logarithmic values of intracranial pressure and the collected time. It was shown that the decreasing rate of intracranial pressure after the volume load >3 ml was much slower than that before the volume load <3 ml. The decrement rate of intracranial pressure of the first stage of increased intracranial pressure was much higher than that of the second stage of increased intracranial pressure. Conclusions The variation of the values of LICP could reflect the values of VICP. The decreasing trend of intracranial pressure after increase of intracranial volume loading was an exponential curve. The loading volume of 3 ml was the breaking point, above which the volume compensation and intracranial compliance decreased greatly.
