主要组织相容性复合体在卵巢上皮性肿瘤中表达及对预后的影响

摘要 目的:探讨主要组织相容性复合体(MHCIb,HLA-DR)、血管内皮生长因子(VEGF)、共刺激分子(hB7-1)在人卵巢上皮性肿瘤组织中的表达与卵巢上皮性肿瘤发生、发展、预后及肿瘤免疫逃逸的关系,为卵巢癌的生物、基因治疗以及预后的评价提供理论依据。方法:选择1999-01/2003-12本院手术切除的人卵巢上皮癌存档标本52块。选择交界性卵巢上皮肿瘤10块,良性卵巢肿瘤10块,正常卵巢组织10块作为对照。采用SP免疫组化技术检测MHCIb,HLA-DR,hB7-1,VEGF在卵巢上皮性肿瘤中的表达。结果:MHCIb,HLA-DR在卵巢上皮癌中的表达分别为14%,59%,与正常卵巢组织及良性肿瘤比较差异均具有显著性意义(P<0.01或P<0.05),其中HLA-DR表达与组织学类型、临床分期有关,低分化腺癌和宫内膜样癌以及I~II期和III~IV期的表达强度相比差异均有明显意义(P<0.01或P<0.05);VEGF抗原在52例卵巢上皮癌的表达阳性率为75%,与正常卵巢组织及良性肿瘤比较差异有显著性意义(P<0.01),且与临床分期有关,I~II期和III~IV期的表达强度相比差异有明显意义(P<0.01);hB7-1抗原在52例卵巢上皮癌的表达阳性率为20%,较正常卵巢组织及良性肿瘤差异均具有显著性意义(P<0.01或P<0.05)。结论:,卵巢上皮癌组织中MHCIb,HLA-DR,hB7-1。 AIM:To investigate the expression of major histocompatibility complexes(MHC Ib,HLA DR),vascular endothelial growth facotor(VEGF)and positive cells costimulatory molecule(hB7 1) in human ovarian carcinoma tissues and their relationships with the occurrence,development,prognosis and immune escape of ovarian epithelial tumors,so as to provide the theoretical bases for the further biology,gene treatment and evaluating prognosis of ovarian carcinoma. METHODS:Fifty two filed cases of human ovarian epithelial carcinoma dissected by operation in our hospital between January 1999 and December 2003 were selected.Another 10 cases of borderline ovarian epithelial tumors,10 cases of benign ovarian tumors and 10 cases of normal ovarian tissues were selected as controls.SP immunocytochemical technique was used to detect the expression of MHCIb,HLA DR,hB7 1 and VEGF in ovarian epithelial tumors. RESULTS:The expressions of MHC Ib and HLA DR in the ovarian epithelial tumors were 14% and 59% respectively,significantly different from those in the normal ovarian tissues and in the benign ovarian tissues(P< 0.01 or P< 0.05);the expression of HLA DR was associated with the histological typing and clinical staging,and its expressive intensity in the low differentiated adenocarcinoma and endometrioid carcinoma was significantly different from that at stage Ⅰ to Ⅱ and stage Ⅲ to Ⅳ (P< 0.01 or P< 0.05).The positive expression rate of VEGF antigen was 75% in the 52 cases of ovarian epithelial carcinoma,significantly different from that in the normal ovarian tissues or benign epithelial carcinoma(P< 0.01);the expression of VEGF antigen was associated with the clinical staging,and its expressive intensity was significantly different between the stage stage Ⅰ to Ⅱ and stage Ⅲ to Ⅳ (P< 0.01).The positive expression rate of hB7 1 antigen was 20% in the 52 cases of ovarian epithelial carcinoma,significantly different from that in the normal ovarian tissues or benign epithelial carcinoma(P< 0.01 or P< 0.05). CONCLUSION:The expression changes of MHC Ib,HLA DR,hB7 1 and VEGF in human ovarian carcinoma tissues may play an important role in initiating auto immunity against ovarian carcinoma;they are associated with the histological typing and staging,and the detection of them is instructive to the determination of occurrence,development and prognosis of ovarian epithelial carcinoma.
