摘要
目的 探讨梅毒患者外周血TH1/TH2淋巴细胞比分布的漂移情况及其与梅毒病程的相关性。方法 采用流式细胞分析技术检测 38例Ⅱ期梅毒患者和 2 0例健康体检者外周血CD3 + 、CD8-T细胞胞内细胞因子IFN γ和IL 4的表达。结果 与正常对照组相比 ,显性梅毒、隐性梅毒患者的TH1细胞数降低 ,TH2细胞数增高 ,TH1/TH2比值降低 ,差异有显著性 (P <0 .0 1) ;而隐性梅毒与显性梅毒比较 ,其TH1淋巴细胞数显著下降 (P <0 .0 5 ) ,TH2和TH1/TH2比值差异均无显著性 (P >0 .0 5 )。结论 梅毒患者TH1型反应模式占弱势状态 ,TH2型反应模式处于优势状态 ,TH1/TH2平衡失调 ,TH1/TH2平衡向TH2方向漂移 ,这可能导致机体不能有效清除TP ,促进疾病向潜伏阶段发展。
OBJECTIVE To discuss the excursion of peripheral blood TH1/TH2 cells in patients with syphilis and to analyze its relativity to the course of syphilis. METHODS Interferon-γ (IFN-γ) and interleukin 4 (IL-4) secreted by CD 3 +, CD 8 -T cells were detected by flow cytometry in 38 patients with secondary syphilis and 20 healthy volunteers, and significance of TH1/TH2 cell ratio in the etiology of syphilis was analyzed. RESULTS Compared with the health control subjects, peripheral blood TH1 cells were significantly decreased in patients with syphilis, while TH2 cells increased. The TH1/TH2 cell ratio was significantly reduced (P<0.01). TH1 cells in patients with dormant syphilis were significantly lower than that in obvious syphilis (P<0.05), TH2 cells and TH1/TH2 cell ratio were not different from that in obvious syphilis (P>0.05). CONCLUSIONS There is a TH1/TH2 imbalance in syphilis patients with a decline of TH1 type cytokines. It might be the mechanism that body doesn′t efficiently eliminate TP, and advance the course progress from primary syphilis to the dormant syphilis.
