不同时间窗Schuell刺激疗法联合高压氧舱治疗对脑梗死所致运动性失语患者言语功能的影响研究 被引量:23

摘要 目的探讨不同时间窗Schuell刺激疗法联合高压氧舱治疗对脑梗死所致运动性失语患者言语功能的影响。方法选择2008年8月—2011年9月我院住院的大动脉粥样硬化型脑梗死后运动性失语患者105例,采用随机数字表分为对照组、早期治疗Ⅰ组(时间窗<24 h)、早期治疗Ⅱ组(24 h≤时间窗<7 d)、早期治疗Ⅲ组(7 d≤时间窗<14 d)和晚期治疗组(时间窗≥14 d),每组21例。对照组予以基础治疗及Schuell刺激疗法,其他各组予以基础治疗及Schuell刺激疗法联合高压氧舱治疗。比较各组治疗后失语症评分总分及复述、说、朗读评分。结果治疗后各组失语症评分总分及复述、说、朗读评分比较,差异均有统计学意义(P<0.05)。早期治疗Ⅰ组、早期治疗Ⅱ组、早期治疗Ⅲ组和晚期治疗组失语症评分总分、复述评分、说评分及朗读评分与对照组比较,差异均有统计学意义(P<0.05);早期治疗Ⅰ组失语症评分总分、复述评分与晚期治疗组比较,差异均有统计学意义(P<0.05)。结论Schuell刺激疗法联合高压氧舱治疗能显著提高运动性失语患者言语表达能力,降低致残率,越早(<24 h)进行康复治疗,患者的言语功能恢复越好。 Objective To evaluate the effectiveness of the combination of Schuell excitation therapy and hyperbaric oxygenation therapy in treating patients with motor aphasia caused by acute ischemic stroke.Methods Totally 105 patients with motor aphasia who were treated in our hospital from August 2008 to September 2011 were equally divided into five groups using the random number table:normal control group,early treatment group Ⅰ(the rehabilitation time window<24 h),early treatment groupⅡ(24 h≤time window<7 d),early treatment group Ⅲ(7 d≤time window<14 d),and late treatment group(time window ≥14 d).The normal control group treated with routine treatment and Schuell excitation therapy,whereas the four treatment groups treated with Schuell excitation therapy,hyperbaric oxygenation therapy,and routine treatment.The severity of aphasia was assessed after treatment.Results The total score and the scores of retelling,speaking,and reading were significantly improved after treatment in all these groups(P<0.05).There were significant differences in early treatment group Ⅰ,Ⅱ,Ⅲ and late treatment groups when comparised to the normal control group(P<0.05).Compared with the late treatment group,the early treatment group Ⅰ had significantly better total score of aphasia and score of retelling(P<0.05).Conclusion The combination of Schuell excitation therapy and hyperbaric oxygenation therapy is effective and safe in improving motor aphasia and reducing disability.Early(<24 h)intervention can achieve better prognosis.
