摘要
目的:探讨氯胺酮对失血性休克大鼠肺组织肿瘤坏死因子-α(TNF-α)﹑核转录因子-κB(NF-κB)的影响。方法:30只成年SD大鼠随机分为3组,对照组(C组)、失血性休克/复苏组(Z组)、失血性休克/复苏联合氯胺酮组(K组)。采用Deree方法建立失血性休克模型。Z组60 min后开始复苏并回输全部血液和2倍失血量的乳酸林格氏液,K组复苏开始前经腹腔注射氯胺酮10 mg/kg。复苏后6 h处死动物取肺组织称重,计算肺组织湿/干重比(W/D)、观察肺组织病理学变化、测定肺组织髓过氧化物酶(myeloperoxidase,MPO)活性,采用免疫组化法检测TNF-α﹑NF-κB的表达。结果:与C组相比,Z组和K组W/D、MPO活性均明显升高(P<0.05);K组与Z组相比上述指标明显下降(P<0.01);K组肺组织TNF-α﹑NF-κB的表达与Z组相比明显下降(P<0.01);光镜下K组肺组织间质水肿、肺泡间隔增宽、中性粒细胞浸润程度明显轻于Z组。结论:氯胺酮可以抑制TNF-α﹑NF-κB表达,减轻肺组织病理学改变,降低MPO活性和肺湿/干比,从而减轻失血性休克大鼠肺损伤。
Objective To investigate the effect of ketamine on tumor necrosis factor α(TNF-α) and nuclear factor κB(NF-κB) of lung in rats model of hemorrhagic shock.Methods Thirty adult SD rats were randomly divided into 3 groups(n=10 each):control group(group C),hemorrhagic shock anabiosis group(group Z) and ketamine group(group K).Hemorrhagic shock model was established by Deree method.Anabiosis of group Z was began after 60 min,at the same time,all blood and ringer lactate solution of 2 double blood loss were transfused.Before anabiosis of group K,ketamine 10 mg/kg was given intraperitoneally.Rats were sacrificed at 6 hours after anabiosis and lung tissue was harvested for weighing.Wet/dry weight ratio of lung tissue was calculated,histopathological change was observed,myeloperoxidase(MPO) activity was measered,TNF-α and NF-κB expression was detected by immunohistochemisty.Results As compared with group C,wet/dry weight ratio and MPO activity in group Z and K obviously increased(P<0.05);the above-mentioned index in group K significantly decreased(P<0.01) as compared with group Z.In group K,TNF-α and NF-κB expression significantly decreased(P<0.01),lung interstitial edema,alveolar interval widening and neutrophil infiltrating obviously was alleviated as compared with group Z.Conclusion Ketamine may inhibit TNF-α and NF-κB expression,alleviate histopathology change,decrease MPO activity and wet/dry weight ratio,so ameliorate pulmonary injury in rats model of hemorrhagic shock.
