摘要
目的探讨碳酸钙D3片联合亮丙瑞林对中枢性性早熟(CPP)女童性激素、骨代谢的影响。方法将2019年1月至2021年1月就诊于无锡市儿童医院的146例中枢性性早熟女童纳入本研究,按随机数表法分组。对照组采用亮丙瑞林治疗,研究组在对照组基础上增用碳酸钙D3片。比较2组生长发育指标、性激素水平、性腺体积、骨代谢等指标。结果研究组治疗后12个月生长速度(GV)和预测成年身高(PAH)均高于对照组(P<0.05)。研究组治疗后3个月卵泡刺激素峰值(PFSH)、黄体生成素峰值(PLH)、雌二醇(E2)均低于对照组(P<0.05)。研究组治疗12个月后卵巢容积、子宫容积、卵泡直径均低于对照组(P<0.05)。研究组治疗12个月后血钙(Ca)、维生素D[25-(OH)D]水平均比对照组高,骨钙素(OST)、I型胶原羧基末端肽(β-CTX)水平均比对照组低(P<0.05)。治疗12个月后研究组不良反应总发生率为8.22%,与对照组6.85%相比,差异无统计学意义(P>0.05)。结论碳酸钙D3片联合亮丙瑞林可有效改善CPP女童的生长发育及血清性激素水平,抑制性腺发育,并有助于调节骨骼代谢,值得临床推广。
Objective To explore the effect of calcium carbonate D3 tablets combined with leuprorelin on sex hormones and bone metabolism in girls with central precocious puberty(CPP).Methods A total of 146 girls with precocious puberty who visited Wuxi Children's Hospital from January 2019 to January 2021 were included in this study and divided into either a control group or a study group.The control group was treated with leuprorelin alone,and the study group was given leuprorelin plus calcium carbonate D3 tablets.Growth index,sex hormone levels,sex gland volume,and bone metabolism were compared between the two groups.Results The 12-month growth rate(GV)and predicted adult height(PAH)in the study group were significantly higher than those in the control group(P<0.05).Peak follitropin beta(PFSH),lutropin alpha(PLH),and estradiol(E2)in the study group were signifiancely lower than those in the control group at 3 months after treatment(P<0.05).Ovarian volume,uterine volume,and follicle diameter in the study group were significantly lower than those of the control group after 12 months of treatment(P<0.05).Blood calcium(Ca)and vitamin D[25-(OH)]were significantly higher and osteocalcin(Ost)and type I collagen carboxyl terminal peptide(β-CTX)were significantly higher in the study group than in the control group(P<0.05)after 12 months of treatment.The total incidence of adverse reactions did not differ significantly between the study group and the control group(8.22%vs 6.85%,respectively,P>0.05).Conclusion Calcium carbonate D3 tablets combined with leuprorelin can effectively improve the growth and development of CPP girls,inhibit the development of the gonads,and help regulate bone metabolism.
