摘要
目的 评价地佐辛复合异丙酚在原发性三叉神经痛射频治疗中的镇痛效果.方法 选取2015年10月至2017年10月就诊于广东省中山市人民医院的原发性三叉神经痛患者60例,按照随机数字表法将其随机分为复合组和异丙酚组,各组30例.复合组采用地佐辛复合异丙酚进行麻醉,静脉推注地佐辛5 mg后,缓慢静脉推注异丙酚2.5~3.5 mg/kg负荷剂量后以1.0~2.5 mg· kg-1·h-1的速率持续输注;异丙酚组采用异丙酚进行麻醉,缓慢静脉推注异丙酚2.5~3.5 mg/kg负荷剂量后以1.0~2.5 mg·kg-1·h-1的速率持续输注;记录两组患者麻醉前、诱导后、热凝时和术后苏醒时平均脉压(MAP)、心率(HR)、血氧饱和度(SpO2),统计异丙酚用量、苏醒时间及术中体动次数、呼吸抑制和心动过缓发生情况.结果 异丙酚组诱导后及热凝时的MAP较复合组下降更明显,差异有统计学意义(P<0.05);异丙酚组的异丙酚用量多于复合组,差异有统计学意义(P<0.05);复合组体动次数、呼吸抑制和心动过缓例数均少于异丙酚组,差异有统计学意义(P<0.05).结论 地佐辛复合异丙酚在原发性三叉神经痛射频治疗时的镇痛效果优于单纯异丙酚静脉麻醉,且不良反应较少.
Objective To observe the efficacy of dezocine combined with propofol for analgesia during radiofrequency thermocoagulation on trigeminal ganglion in the treatment of patients with primary trigeminal neuralgia.Methods Sixty patients with trigeminal neuralgia in Zhongshan People's Hospital of Guangdong Province from October 2015 to October 2017,undergoing radiofrequency thermocoagulation on trigeminal ganglion,were randomly divided into compound group(30 cases)and propofol group(30 cases).Patients received anesthesia with dezocine combined with propofol in the compound group,intravenous dezocine 5 mg at first,then propofol 2.5-3.5 mg/kg was followed by continuous infusion at the rate of 1.0-2.5 mg·kg-1·h-1 after 1 min,While patients received propofol only in the propofol group.MAP,HR and SpO2,propofol dosage,analgesia time,the incidences of intraoperative respiratory depression and bradycardia were recorded in the two groups.Results MAP was lower significantly in the propofol group than that in the compound group duringanesthesia induction and operative thermocoagulation(P<0.05).The dosage of propofol was higher significantly in the propofol group than that in the compound group(P<0.05).The incidences of body movement,respiratory depression and bradycardia were less significantly in the compound group than those in the propofol group(P<0.05).Conclusion Dezocine combined with propofol is superior to propofol only for analgesia during radiofrequency thermocoagulation on trigeminal ganglion in the treatment of patients with primary trigeminal neuralgia,with less adverse reactions.
