摘要
目的总结腹腔镜胆囊切除术胆道损伤发生的原因、处理方法及预防措施。方法回顾我院2009年1月~2012年12月共实施Lc手术273例,所用的手术方法是四孔法,7例腹腔镜胆道损伤患者的临床资料,分析其发生的原因、部位、处理方法和治疗效果。结果经中转开腹胆管壁修补4例。3例术后出现胆漏经腹腔镜及术中置负压引流管,6例痊愈,1例死亡。结论腹腔镜胆囊切除术术前正确选择病例,术中规范操作,是防止胆道损伤的有效措施;胆道损伤早期发现和正确处理对预后十分重要。
Objective To summarize the experience of laparoscopic cholecystectomy for bile duct injury reasons,treatment methods and preventivemeasures.Methods during 2009 January to 2012 in our hospital in Decemberhas implemented a total of 273 cases of Lc operation,operation method is used by the four hole method,the clinical data of 7 cases of patients with laparoscopicbile duct injury,analysis methods and therapeutic efficacy of the reasons for its occurrence,location,treatment.Results After open abdominal wall repair of bile duct in 4 cases.Bile leakage after laparoscopic operation and the mid drainage tube in 3 cases,6 cases were cured,1 cases died.Conclusion Laparoscopic cholecystectomy for proper selection of the patients before operation,standard operation,is an effective measure to prevent bile duct injury;bile duct injuryearly discovery and correct treatment is important for prognosis.
