摘要
目的 探讨慢性自发性荨麻疹(CSU)患者外周血辅助性T细胞17(Th17)、调节性T细胞(Treg)及相关细胞因子水平变化价值.方法 选择2014年1月至12月CSU患者120例,其中活动期65例,静止期55例,流式细胞仪对外周血Th17、Treg细胞测定,酶联免疫吸附法(ELISA)测定血清白介素-17(IL-17)、白介素-21(IL-21)、转化生长因子-β(TGF-β)水平,以同期健康体检50例为对照.Spearman相关性分析血清IL-17、IL-21、TGF-β水平与症状评分的相关性.结果 CSU活动期外周血Th17细胞比例、血清IL-17、IL-21水平均显著高于CSU静止期、对照组,Treg细胞比例、血清TGF-β水平均显著低于CSU静止期、对照组,差异有统计学意义(P<0.05),CSU静止期与对照组外周血Th17、Treg细胞比例、血清IL-17、IL-21、TGF-β水平比较差异均无统计学意义(P>0.05);血清IL-17、IL-21水平与症状评分显著正相关(P<0.05),TGF-β水平与症状评分无显著相关性(P>0.05).结论 Th17/Treg细胞失衡可能参与CSU发病过程,血清IL-17、IL-35水平可作为CSU疾病活动评估重要指标.
Objective To study the value of changes in levels of peripheral blood helper T cell 17(Th17),regulatory T cells(Treg) and related cytokines in patients with chronic spontaneous urticaria(CSU). Methods A total of 120 patients with CSU who were treated in the hospital from January 2014 to December 2014 were enrolled in the study,including 65 cases in active phase and 55 cases in stationary phase. Flow cytometry was used to detect peripheral blood Th17 and Treg cells. The levels of serum interleukin-17(IL-17),interleukin-21(IL-21)and transforming growth factor-β(TGF-β)were determined by enzyme linked immunosorbent assay(ELISA). At the same time,50 normal persons were selected as controls. The correlation of levels of serum IL-17,IL-21 and TGF-β with symptom scores was analyzed by Spearman correlation analysis. Results The ratio of peripheral blood Th17 cells and serum IL-17 and IL-21 levels were significantly higher,while the ratio of Treg cells and serum TGF-β levels were significantly lower in CSU active phase than in CSU stationary phase or in the control group (P<0.05),without significant difference between CSU active phase and the control group(P>0.05). The levels of serum IL-17 and IL-21 were positively correlated with symptom scores(P<0.05),but there was no significant correlation between the level of TGF-β and symptom scores(P>0.05). Conclusion The imbalance of Th17/Treg cells may be involved in the pathogenesis of CSU,and levels of serum IL-17 and IL-35 can be used as important indexes for evaluating the activity of CSU.
