摘要
目的:探讨经腹全子宫与次全子宫切除术对子宫肌瘤患者盆底功能及性功能的影响.方法:选取2013年2月~2018年9月我科收治的子宫肌瘤患者80例,根据手术方式不同将患者分为实验组(n=50)和对照组(n=30),对照组行经腹全子宫切除术,实验组行经腹次全子宫切除术.比较2组患者手术前后血清性激素水平、盆底功能及性功能.结果:与术前比较,术后2组患者血清LH、FSH水平均明显升高,但实验组明显低于对照组;2组E2水平明显降低,但实验组明显高于对照组.与术前比较,术后3个月2组患者PFDI-20各项评分均明显降低,且实验组明显低于对照组.与术前比较,术后3个月2组患者性欲、性交频率、性高潮、性交流、性交舒适程度及夫妻关系评分均明显升高,且实验组明显高于对照组.结论:相较于全子宫切除术,经腹次全子宫切除术治疗子宫肌瘤对患者术后盆底功能及性激素的影响更小,可在一定程度上改善患者性生活质量.
Objective To investigate the influence of total and subtotal hysterectomy on pelvic floor function and sexual function in patients with uterine fibroids.Methods 80 case of patients with uterine fibroids admitted to our department from February 2013 to September 2018 were selected and they were divided into experiment group(n=50)and control group(n=30)according to different surgical methods.The control group underwent transabdominal total hysterectomy,and the experiment group underwent transabdominal subtotal hysterectomy.The serum levels of sex hormone,pelvic floor function and sexual function were compared before and after surgery in the 2 groups.Results Compared with preoperation,the serum levels of LH and FSH significantly increased at postoperation in the 2 groups,but the experiment group was significantly lower than those in the control group;the levels of E2 significantly decreased in the 2 groups,but the experiment group was significantly higher than the control group.Compared with preoperation,the PFDI-20 scores significantly decreased 3 months after operation in the 2 groups,and the experiment group was significantly lower than those in the control group.Compared with preoperation,the scores of sexual desire,sexual frequency,orgasm,sexual communication,sexual intercourse comfort and relationship between husband and wife significantly increased 3 months after operation in the 2 groups,and the experiment group was significantly higher than the control group.Conclusion Compared with total hysterectomy,transabdominal subtotal hysterectomy in treatment of uterine fibroids has a smaller influence on postoperative pelvic floor function and sex hormones,which can improve the quality of sexual life of patients to some extent.
