万古霉素在颅脑损伤术后颅内感染患者中的应用 被引量:1

Application of Vancomycin in Patients with Intracranial Infection after Craniocerebral Injury

摘要 目的探讨万古霉素治疗颅脑损伤术后颅内感染患者的临床效果.方法选取该院2018年8月—2020年8月收治的96例颅脑损伤术后颅内感染患者为研究对象,采用掷硬币法分为观察组(48例)和对照组(48例).两组患者均接受术后常规治疗,在此基础上,对照组鞘内注射美罗培南治疗,观察组鞘内注射万古霉素治疗.比较两组的症状消退时间及住院时间,治疗前后的各项炎症指标水平及治疗期间的不良反应发生率.结果观察组的体温恢复时间、头痛消失时间、脑脊液白蛋白恢复时间、住院时间均均短于对照组,组间差异有统计学意义(P<0.05).治疗后,两组患者的各项炎症指标水平均低于治疗前,且观察组患者的各项指标水平均低于对照组,组间差异有统计学意义(P<0.05).对照组的不良反应发生率为6.25%,观察组的不良反应发生率为14.58%,两组比较,差异无统计学意义(P>0.05).结论采用万古霉素治疗颅脑损伤术后颅内感染可促进患者病情康复,缩短住院时间,减轻炎症反应. Objective To investigate the clinical effect of vancomycin on patients with intracranial infection after craniocerebral injury.Methods A total of 96 patients with intracranial infection after craniocerebral injury admitted to the hospital from August 2018 to August 2020 were selected as the research objects.They were divided into an observation group(48 cases)and a control group(48 cases)by coin toss method.Both groups received routine postoperative treatment.On this basis,the control group was treated with intrathecal injection of meropenem,and the observation group was treated with intrathecal injection of vancomycin.The time of symptom resolution,length of hospital stay,levels of inflammatory indicators before and after treatment and incidence of adverse reactions during treatment were compared between the two groups.Results The body temperature recovery time,headache recovery time,cerebrospinal fluid albumin recovery time and hospital stay time in the observation group were shorter than those in the control group,and the differences between the groups were statistically significant(P<0.05).After treatment,the levels of all inflammatory indexes in the two groups were lower than before,and the levels of all inflammatory indexes in observation group were lower than those in the control group,the differences between the groups were statistically significant(P<0.05).The incidence of adverse reactions was 6.25%in the control group and 14.58%in the observation group,and there was no statistical significance between the two groups(P>0.05).Conclusion Vancomycin treatment of intracranial infection after craniocerebral injury can promote the recovery of patients,shorten the hospital stay,reduce the inflammatory response.
