3D打印技术在人工全髋关节置换术中的临床应用效果分析

摘要 目的探讨3D打印技术在人工全髋关节置换术中的应用效果.方法以2019年4月—2020年7月我院收治的40例人工全髋关节置换手术患者为研究对象,依据治疗方法不同分为对照组(20例)与治疗组(20例).对照组单纯采用常规后外侧入路人工全髋关节置换术治疗,治疗组在常规后外侧入路人工全髋关节置换术治疗中应用3D打印技术.测量治疗组患者3D打印模型还原的髋臼假体大小与髋臼中心点下移值,并与术后实际测量值进行对比.记录两组基本手术指标,通过影像学检查评估两组治疗后髋臼假体位置与髋臼杯稳定性,分别于治疗前及治疗后不同时点,采用疼痛数字评分法(NRS)评估两组的髋关节疼痛情况,采用Harris髋关节功能评分量表(HHS)评估两组的髋关节功能.结果治疗组基于术前3D打印模型测量的髋臼假体大小和中心点下移值与术后实际测量值比较,差异无统计学意义(P>0.05).治疗组手术时间短于对照组,术中出血量与术后引流量均少于对照组,差异有统计学意义(P<0.05).治疗组术后髋臼杯与骨面之间未见透亮线,髋臼杯稳定性良好.治疗组治疗后NRS评分低于对照组,HHS评分高于对照组,差异有统计学意义(P<0.05).结论3D打印技术应用于人工全髋关节置换术中,假体安放精准度高,可以缩短手术时间,减少术中出血量与术后髋关节疼痛,促进髋关节功能恢复. Objective To explore the application effect of 3D printing technology in artificial total hip arthroplasty.Methods A total of 40 patients with total hip arthroplasty who were treated in our hospital from April 2019 to July 2020 were selected as the research objects,and they were divided into the control group(20 cases)and the treatment group(20 cases)according to the different treatment methods.The control group was treated with conventional total hip arthroplasty through posterolateral approach alone,while the treatment group was treated with 3D printing technology combined with conventional total hip arthroplasty through posterolateral approach.The size of the acetabular prosthesis and the downward displacement of the center point of the acetabulum restored by the 3D printing model of the patients in the treatment group were measured,and compared with the actual measurement value after operation,the two groups of basic surgical indicators were recorded,the position of the acetabular prosthesis and the stability of the acetabular cup after treatment in the two groups were evaluated by imaging examination,before treatment and at different time points after treatment,the numerical rating scale(NRS)was used to evaluate the hip joint pain in the two groups,Harris hip score(HHS)was used to evaluate hip function in both groups.Results There was no statistically significant difference in the size of the acetabular prosthesis and the downward displacement of the center point measured based on the preoperative 3D printing model in the treatment group and the actual measured value after the operation(P>0.05).The operation time of the treatment group was shorter than that of the control group,and the intraoperative blood loss and postoperative drainage were less than those of the control group,and the differences were statistically significant(P<0.05).There was no translucent line between the acetabular cup and the bone surface in the treatment group,and the acetabular cup was stable.After treatment,the NRS score in the treatment group was lower than that in the control group,and the HHS score was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusion The application of 3D printing technology in artificial total hip replacement has high prosthesis placement accuracy,which can shorten the operation time,reduce intraoperative blood loss and postoperative hip pain,and promote hip function recovery.
