电针四渎、外关穴治疗脑卒中后手功能障碍的临床疗效观察 被引量:1

Observation on the Clinical Effect of Electroacupuncture at Sidu and Waiguan Points on Hand Dysfunction after Stroke

摘要 目的观察电针四渎和外关穴治疗脑卒中后手功能障碍的临床疗效.方法选取该院2017年3月-2019年10月收治的脑卒中后手功能障碍患者共计80例为研究对象,随机分为电针组和对照组,每组40例.电针组采用电针四渎和外关穴治疗,对照组采用中频电刺激四渎和外关穴治疗.比较两组患者治疗前后的患侧前臂屈伸肌群表面肌电均方根值(root mean square,RMS)及Fugl-Meyer运动功能评定(Fugl-Meyer assessment,FMA)手功能评分.结果治疗前,两组的患侧前臂屈肌群、伸肌群表面肌电RMS及FMA手功能评分对比,组间差异无统计学意义(P>0.05).治疗后,两组的患侧前臂屈肌群、伸肌群表面肌电RMS及FMA手功能评分均较各组治疗前明显升高,差异有统计学意义(P<0.05);两组的患侧前臂屈肌群表面肌电RMS对比,差异无统计学意义(P>0.05);电针组的患侧前臂伸肌群表面肌电RMS、FMA手功能评分均高于对照组,组间差异有统计学意义(P<0.05).电针组的治疗总有效率92.50%高于对照组的75.00%,差异有统计学意义(P<0.05).结论与单纯中频电刺激四渎和外关穴相比,电针四渎和外关穴治疗脑卒中后手功能障碍的疗效更为显著,值得临床推广应用,其具体作用机制有待进一步研究. Objective To observe the clinical efficacy of electroacupuncture at Sidu and Waiguan points in the treatment of hand dysfunction after stroke.Methods A total of 80 patients with post-stroke hand dysfunction admitted to the hospital from March 2017 to October 2019 were selected as the research objects,and they were randomly divided into an electroacupuncture group and a control group,with 40 cases in each group.The electroacupuncture group was treated with electroacupuncture at Sidu and Waiguan points,and the control group was treated with intermediate frequency electrical stimulation at Sidu and Waiguan points.The root mean square(RMS)of the surface electromyography of the affected forearm flexors and extensors and Fugl-Meyer assessment(FMA)hand function scores of the two groups before and after treatment were compared.Results Before treatment,the comparison of the RMS of the surface electromyography of the affected forearm flexors and extensorsand and FMA hand function scores between the two groups showed no significant difference(P>0.05).After treatment,the RMS of the surface electromyography of the affected forearm flexors and extensors and FMA hand function scores of the two groups were significantly higher than those of each group before treatment,and the difference was statistically significant(P<0.05);there was no significant difference in RMS of the surface electromyography of affected forearm flexors between the two groups(P>0.05);the RMS of the surface electromyography of the affected forearm extensor and FMA hand function scores in the electroacupuncture group were higher than those of the control group,and the differences between the groups were statistically significant(P<0.05).The total effective rate of treatment in the electroacupuncture group was 92.50%,which was higher than 75.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with simple intermediate frequency electrical stimulation of Sidu and Waiguan points,electroacupuncture at Sidu and Waiguan points is more effective in treating hand dysfunction after stroke.It is worthy of clinical application and its specific mechanism needs further study.
