摘要
目的 探究下肢动脉硬化闭塞症(arteriosclerosis obliterans of lower extremity,LEASO)患者应用TurboHawk斑块旋切术治疗的疗效.方法 选取该院2017年1月—2020年12月收治的98例LEASO患者,按随机数字表法分成研究组(n=49)、常规组(n=49).常规组接受经皮腔内球囊扩张血管成形术及经皮腔内支架植入术治疗,研究组接受TurboHawk斑块旋切术治疗.比较两组手术效果;术前、术后1 d的患侧肢体循环情况;术前、术后7、14 d的Fugl-Meyer运动功能评定(Fugl-Meyer assessment,FMA)评分;术前、术后3 d的趾肱指数(toe brachial index,TBI)、踝肱指数(ankle brachial index,ABI).结果 研究组总有效率为97.96%较常规组的81.63%高,差异有统计学意义(P<0.05).术后1 d,研究组的患肢皮肤温度较常规组高,血流速度较常规组快,患肢周径增长长度较常规组短,组间差异有统计学意义(P<0.05).术后7、14 d,研究组的FMA评分较常规组高,组间差异有统计学意义(P<0.05).术后3 d,研究组的ABI、TBI较常规组高,组间差异有统计学意义(P<0.05).结论 LEASO患者应用TurboHawk斑块旋切术治疗的效果明显,可有效改善患侧肢体循环情况,促进肢体功能恢复.
Objective To explore the efficacy of TurboHawk atherectomy for patients with arteriosclerosis obliterans of lower extremity(LEASO).Methods A total of 98 LEASO patients admitted to the hospital from January 2017 to December 2020 were selected and divided into study group(n=49)and routine group(n=49)according to the random number table method.The routine group received percutaneous transluminal angioplasty and prcutaneous transluminal stenting treatments,the study group received TurboHawk atherectomy treatment.The surgical effect,the circulation of the affected limb before and 1 d after the operation,Fugl-Meyer assessment(FMA)scores before the operation,7 and 14 d after the operation,and the toe brachial index before the operation and 3 d after the operation(TBI)and ankle brachial index(ABI)of the two groups are compared.Results The total effective rate in the study group was 97.96%,which was higher than 81.63%in the routine group,and the difference was statistically significant(P<0.05).One day after operation,the skin temperature of the affected limb in the study group was higher than that in the routine group,the blood flow rate was faster than that in the routine group,and the circumference of the affected limb was shorter than that in the routine group,and the difference between the groups was statistically significant(P<0.05).At 7 and 14 days after operation,the FMA score of the study group was higher than that of the routine group,and the difference between the groups was statistically significant(P<0.05).At 3 days after operation,the ABI and TBI of the study group were higher than those of the routine group,and the difference between the groups was statistically significant(P<0.05).Conclusion The effect of TurboHawk atherectomy for LEASO patients is obvious,which can effectively improve the circulation of the affected limb and promote the recovery of limb function.
