盆底康复锻炼联合电刺激生物反馈对子宫切除术后盆底功能障碍性疾病的预防效果研究 被引量:7

摘要 目的 探讨盆底康复锻炼联合电刺激生物反馈对子宫切除术后盆底功能障碍性疾病的预防效果.方法 选取2019年1月—2020年4月于该院行子宫切除术的80例患者为研究对象,按照随机数字表法将患者分为研究组与对照组,各40例.对照组术后仅接受Kegel运动盆底康复锻炼,研究组在对照组的基础上联合电刺激生物反馈.比较两组术后3、6、12个月的盆底肌力分级、压力性尿失禁发生率及漏尿量、盆底功能影响问卷简表(pelvic floor impact questionnaire-7,PFIQ-7)得分,并比较两组术后6、12个月的盆腔器官脱垂-尿失禁性功能问卷(pelvic organ prolapse/incontinence sexual function questionnaire-12,PISQ-12)评分.结果 术后3个月,两组盆底肌力Ⅲ~Ⅴ级患者占比、压力性尿失禁发生率及漏尿量比较,组间差异无统计学意义(P>0.05);术后6、12个月,两组盆底肌力Ⅲ~Ⅴ级患者占比均高于同组术后3个月,且研究组术后6个月盆底肌力Ⅲ~Ⅴ级患者占比高于对照组,组间差异有统计学意义(P<0.05).研究组在术后6~12个月的压力性尿失禁及漏尿量均低于对照组,组间差异有统计学意义(P<0.05).术后3个月,两组PFIQ-7评分比较,差异无统计学意义(P<0.05);在术后6、12个月,研究组PFIQ-7评分低于对照组,PISQ-12评分高于对照组,组间差异有统计学意义(P<0.05).结论 予以子宫切除术后患者盆底功能锻炼联合电刺激生物反馈能改善其盆底肌力,促进盆底组织修复,提高生活质量,减少盆底疾病的发生,值得临床应用. Objective To explore the preventive effect of pelvic floor rehabilitation exercise combined with electrical stimulation biofeedback on pelvic floor dysfunction diseases after hysterectomy.Methods 80 patients who underwent hysterectomy in this hospital from January 2019 to April 2020 were selected as the research objects.According to the random number table method,the patients were divided into a study group and a control group,each group with 40 cases.The control group only received Kegel exercise pelvic floor rehabilitation exercises after the operation,and the study group combined electrical stimulation biofeedback on the basis of the control group.The pelvic floor muscle strength classification,stress urinary incontinence incidence and urine leakage,and pelvic floor impact questionnaire-7(PFIQ-7)scores were compared between the two groups at 3,6,and 12 months after surgery,and the scores of pelvic organ prolapse/incontinence sexual function questionnaire-12(PISQ-12)were compared between the two groups at 6 and 12 months after surgery.Results 3 months after the operation,there was no significant difference between the two groups in the proportion of patients with pelvic floor muscle strength gradeⅢ~Ⅴ,the incidence of stress urinary incontinence,and the volume of urine leakage(P>0.05);at 6 and 12 months after surgery,the proportion of patients with pelvic floor muscle strength of grade III~V in the two groups was higher than that of the same group at 3 months after surgery,and the proportion of patients with pelvic floor muscle strength groupⅢ~Ⅴin the study group was higher than that in the control group at 6 months after surgery,the differences between the groups were statistically significant(P<0.05).However,the stress urinary incontinence and urine leakage in the study group were lower than those in the control group at 6-12 months after surgery,and the difference between the groups was statistically significant(P<0.05).3 months after operation,there was no statistically significant difference in PFIQ-7 scores between the two groups(P<0.05);at 6 and 12 months after surgery,the PFIQ-7 score of the study group was lower than that of the control group,and the PISQ-12 score was higher than that of the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Pelvic floor function exercise combined with electrical stimulation biofeedback after hysterectomy can improve pelvic floor muscle strength,promote pelvic floor tissue repair,improve quality of life,and reduce the occurrence of pelvic floor diseases.It is worthy of clinical application.
