摘要
                
                    目的探讨根治性胃癌术后患者采用肠内营养支持对改善胃肠功能、营养状态、应激及炎性反应的作用.方法选取本院2018年1月—2020年12月收治的行根治性胃癌术患者82例为研究对象,以随机数表法分组,对照组(n=41)采用肠外营养支持,观察组(n=41)采用肠内营养支持.对比两组术后的胃肠功能恢复情况,检测患者的白蛋白、前白蛋白、血清总蛋白水平以评估营养状态,并比较两组患者的C反应蛋白(CRP)、白细胞介素-6(IL-6)、皮质醇(Cor)水平.结果观察组的术后肠鸣音恢复、排气、排便、经口进食时间分别为(62.45±7.39)h、(67.46±10.14)h、(80.05±9.54)h、(84.29±10.40)h,均短于对照组,组间差异有统计学意义(P<0.05).观察组术后7 d的白蛋白、前白蛋白、血清总蛋白水平分别为(36.52±2.61)g/L、(232.28±17.61)mg/L、(64.42±3.38)g/L,均高于对照组,组间差异有统计学意义(P<0.05).观察组术后7 d的CRP、IL-6、Cor水平分别为(9.36±3.04)mg/L、(44.25±7.16)pg/mL、(165.42±11.50)pg/mL,均低于对照组,组间差异有统计学意义(P<0.05).结论在根治性胃癌术后采用肠内营养支持能促进患者胃肠功能恢复,提高机体营养水平,减轻应激及炎性反应,值得推广.
                
                Objective To investigate the effect of enteral nutritional support on improving gastrointestinal function,nutritional status,stress and inflammatory response in patients with radical gastric cancer.Methods A total of 82 postoperative patients with radical gastric cancer treated in our hospital from January 2018 to December 2020 were selected as the research objects,and were divided into two groups by random number table method.The control group(n=41)received parenteral nutrition support,and the observation group(n=41)received parenteral nutrition support.The postoperative gastrointestinal function recovery of the two groups was compared,the levels of albumin,prealbumin and serum total protein were detected to evaluate the nutritional status of the patients,and the levels of C-reactive protein(CRP),interleukin-6(IL-6)and cortisol(Cor)were compared.Results The recovery time of bowel sounds,exhaust,defecation and oral feeding in the observation group were(62.45±7.39)h,(67.46±10.14)h,(80.05±9.54)h,(84.29±10.40)h,respectively,which were shorter than those in the control group,and the differences between the groups were statistically significant(P<0.05).The levels of albumin,prealbumin and serum total protein in the observation group 7 days after operation were(36.52±2.61)g/L,(232.28±17.61)mg/L and(64.42±3.38)g/L,respectively,which were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).The levels of CRP,IL-6 and Cor in the observation group 7 days after operation were(9.36±3.04)mg/L,(44.25±7.16)pg/mL and(165.42±11.50)pg/mL,respectively,lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Enteral nutritional support after radical gastrectomy can promote the recovery of gastrointestinal function,improve the nutritional level of the body,reduce stress and inflammatory response,which is worthy of promotion.
